EE WAS CUTTING RUBBER WITH RAZOR KNIFE. KNIFE SLIPPED AND CUT LEFT THUMB. HE WAS TAKEN TO HOSPITAL FOR STITCHES AND RETURNED TO WORK SAME DAY.

EE WAS GRINDING ON JAW DIE THAT WAS BEING HELD BY CRANE THE LIFTING EYE BROKE FREE ALLOWING JAW DIE TO FALL ON EE FEET. EMERGENCY WAS CALLED AND EE WAS TAKEN TO HOSPITAL.

EE WAS OPERATING CRANE WHEN HE BACKED DOWN AN INCLINE AND THE BRAKES FAILED. HE TURNED THE WHEEL SO THE CRANE WOULD HIT THE SAFETY BERM. IT DID HIT THE BERM, BUT THE CRANE ROLLED ON ITS SIDE. THE EE JUMPED FROM THE CRANE BEFORE IT ROLLED TO ITS SIDE.

EE WAS WORKING ON SECONDARY PART OF CRUSHING PLANT WHEN HE NOTICED AN OVERSIZED ROCK ONT HE CONVEYOR. HE ATTEMPTED TO REMOVE IT FROM THE MOVING CONVEYOR. HIS FINGER WAS PINCHED BETWEEN THE ROC K AND THE STRUCTURE.
