When pulling the center bar out, the miners lost their grip on the bar and dropped it on a miners foot just above the steel toe portion. X-rays were done at the time and no fractures were detected and fully released. The miner was still having pain and sent for a follow up on 1/11/25, a x-ray was redone and small fracture found.

Screwing 2 studs together to adjoin the walls at the corner, drill slipped from the screw head, causing impact with right hand between middle and pointer fingers, causing laceration requiring 1 stitch.

Changing media panels in the 25lb 10x24 screen deck. While trying to get panel in place, EE felt left shoulder discomfort. EE did not think it was serious, continued working. The following day (Sunday) EE had pain & minimal ROM. EE went to the ER and then text plant manager.

Employee was installing a chute on the #2 crusher. The chute swung back and employee's left hand and wrist was crushed. Employee was taken to the ER via ambulance. Was released. Was seen by an Orthopedic Specialist. Was given a splint and is restricted.

Employee was working on a crusher and removed a wear plate. The employee positioned the wear plate next to him, which created a tight spot for him to work in. He was using a hammer to clean off old material inside the crusher when he struck the old wear plate next to him causing a laceration. He was not wearing gloves at the time of the incident.
