Employee was struck on the right knee by a piece of belt structure.

Employee was hanging miner cable when they slipped on a small piece of slate rock and fractured the outermost distal toe which resulted in lost workdays.

Employee was operating a continuous miner when EE was struck by a roof bolter trailing cable that was knocked down by a rock that was coming out of the conveyor chain of the miner which resulted in a laceration that required 5 sutures. No lost or restricted work days.

Employee was trying to install a bottom roller when EE crawled underneath the belt to use EE's back to lift the belt which resulted in a sprain/strain injury and lost work days.

Employee was removing a spreader bar from the boom of the continuous miner when a sharp piece of metal on the spreader bar cut employee's hand resulting in a laceration that required 3 sutures. No Lost or Restricted work days.

Injured employee was hanging continuous miner cable in the #6 entry when EE heard a pop in the right elbow which resulted in a sprain / strain type injury and lost work days.

Employee was operating a roof bolter when ee tried to catch drill steel that was falling out of hole and struck left hand against the drill pot causing left pinky finger to become dislocated. This did not result in any lost or restricted days.

Employee stated ee was shoveling the belt line on the Byrd #1 belt at Break 0 when a piece of slate rock fell from the mine roof and struck the employee on the right forearm and left ring finger. The rock measured approximately 2.5' in length X 1.5' in width X 1' in thickness. NOTE: This employee didn't start missing work until 4/2/2019.

Employee was operating a roof bolter machine when a rock fell on its edge and fell over onto EE's left foot which resulted in a contusion and restricted work days.

Employee stated was installing a rib bolt when a sharp piece of slate from rib fell and struck employee above the right eye resulting in a laceration that required sutures. Employee did not have any lost or restricted days.

Employee was operating a roof bolter machine when EE walked beside of the bolter to install line curtain. A piece of rock fell from the mine roof/rib and struck EE on the right shoulder causing EE to fall with the rock landing on EE's right leg. Employee was diagnosed with a fracture to the tip of EE's right shoulder clavicle bone which resulted in restricted days.

Employee states that ee felt pain in thigh area of left leg while putting installing center line in #6 entry. Employee states that ee stepped off a ledge that was 9 inches in depth. Employee was seen by a physician and was given restricted duty work.

Employee was walking to check a pump in the return when they slipped on a slate rock, while trying not to fall they twisted the left knee and heard a popping noise. Employee finished shift and returned to work with no restrictions for next schedule shift. On 6/1/2018 employee had knee surgery and began missing work at which time this incident became reportable.

Employee states that EE was struck with a continuous miner trailing cable that was torn loose from the rope hangers when the cable was caught by the roof bolter and was pulled, breaking the rope hangers.

Employee Stated that EE was walking from #4 entry to # 6 entry EE was proceeding through a set of fly pads in the number 5 entry while looking at the mine roof EE steeped in a hole and twisted EE's left knee.

Employee stepped in a hole walking across the section employee was seen by a doctor and released to full duty. Employee was refereed to a specialist and was scheduled to have surgery on March 5th employee returned to work on restricted duty.

Employee stated there was a rock fouled underneath the chain of a shuttle car. In order to remove it employee was striking the pin out that holds the flight in place. In doing so the flight broke free and struck employee in the face causing a laceration that required several stitches.

Injured employee stated ee and some other employees were attempting to remove a cutter motor using a chain and scoop. In the process of pulling a bolt broke off the cutter motor and struck the injured employee in the abdomen causing a contusion. Employee was given modified duty. Note Employee was not in the line of fire.

Employee stated that employee was climbing into a scoop in the #7 entry. As employee started to climb into the scoop employee lost employee's balance and fell forward striking employee's hand against the bell that is mounted in the operators deck. Employee was placed under restricted duty on 10/30/2017.

Employee stated ee was hauling out of #6 entry when ee ran over a rock that had fallen in the roadway from between bolts. Employee stated ee jammed shoulders and neck when ee came in contact with the canopy. Employee sought medical attention on 10/23/2017 and was placed off of work.

Employee stated they were attempting to drill a hole when they pinched their left pinky finger between the wrench and chuck as they raised the pot up. Employee received stitches.

Employee stated EE was helping roll a scrubber motor into a scoop bucket, employee stated EE lacerated palm on a flange while attempting to load motor. Employee received sutures in palm.

Employee stated EE was holding a knocker while attempting to repair a splice while another employee struck the hammer on the knocker. The employee missed the knocker and struck the injured employee on the left index finger.

Employee stated that employee was carrying a roll of fly pad and slipped and fell back landing on the roll of fly pad straining employee's lower back.

Employee stated they lifted miner cable and water line and stepped forward and felt a sharp pain in the right knee. Employee returned to work on 4-28-2017 on light duty. On 5-1-2017 employee was placed off work under doctors orders.

Employee stated ee was driving wedges when setting a feeder. Employee struck left index finger with the hammer. The blow caused a blood blister which later became infected. Employee was placed on light duty on 4-16-2017.

Employee was dismounting the cat 83005 fork lift, employee stated they slipped while dismounting the fork lift and was not able to grab the handle. Employee fell on right side of body injuring right elbow, right shoulder and back

Employee was walking in front of the section feeder, and stooped over to avoid contcting a belt hanger. While walking, leg twisted causing employee to pull a muscle in upper right leg.

At 10:10am Airway Examiner discovered a roof fall in #6 entry on Byrd Mains #2. The fall fell above the ancourage point and measured 45'x19.5'x5' thick. The fall was dangered off and lifeline was re-routed to #7 entry to provide a continuous means of egress from section. Area was previously mined 8 years ago and 4' fully grouted bolts utilized.

Employee was dismounting a scoop. When EE was standing EE's foot went in a hole causing employee to fall forward twisting EE's ankle. EE reported to work on EE's next scheduled shift on restricted duty.

EE was shoveling gob at take-up on Buckshot 4 head. EE had a spasm in EE's back causing EE to fall forward striking EE's ribs. EE was treated for back spasms and bruised ribs. EE returned for EE's next scheduled shift on restricted duty.

A roof fall was discovered on 6-28-2016 located at Buckshot #1 break 29 Spad 6763 in the #6 entry. The fall measured approx 70' long X 19' Wide X 5' high. The area was dangered off and all approaches breakered off. IT will not be cleaned up. The area was originally mined in January 2012.

Employee stated - noticed a rib roll had dislodged a timber. The timber was 6-1/2' tall and EE stood the timber back up to set into place. While lifting EE felt a sharp persistent pain in left groin. Employee went to the doctor who suggested stretching and exercise. EE was released to restricted duty and returned to next scheduled shift.

Employee stated that EE noticed a tightness in arm at the end of shift. EE was unable to identify an event during the shift that would result in an injury. EE continued to work without missing a shift. EE later went to the doctor and needed surgery. EE worked up until surgery on May 25, 2016 and returned to work on light duty.

EE was utilizing a 4" cordless Dewalt angle grinder to remove a damaged lock from a hasp. Was holding the hasp with a pair of channel locks while cutting the lock. Grinder fowled and kicked back cutting through leather glove causing a laceration to left thumb. Laceration required 6 stitches and the ee was placed on light duty to keep the cut clean.

The employee was using a ratchet to pull in a conveyor chain on a shuttle car. The Chain fouled, so the employee attempted to release the chain ratchet. When EE reversed the ratchet, the handle turned loose and the employee suddenly stepped back twisting ankle. EE has been allowed to work light duty.

Mine lost power the morning of the incident. The Motor crew was attempting to come outside and was unlatching elec door to move motor through the door. One jack was unhooked while the second door was being unlatched. The mine fan power was restored and the air current slammed the door shut striking the injured miners arm. EE has been allowed to work light duty.

The Buckshot crew left the section on Company #714 track mounted mantrip. One of the section employees placed a used piece of drill steel on top of the mantrip. When the mantrip pulled through the section airlock doors, the steel caught the door and spun around striking the injured employee.

Employee traveled to #6 Right of West Mains to check #6 pump. EE traveled through a mandoor and crouched down due to the low cavity height. The walkway had ruts from equipment EE stepped on the peak of a rut spraining EE's right knee. EE worked regular duties/shift and had surgery to repair meniscus on 8/18/2016. EE returned on EE's next scheduled shift on restricted duty.

An unplanned roof fall was discovered near Spad #11463 at the mouth of the West Mains 6 left bleeder panel. The fall measured approximately 20' long and 40' wide and fell just above the 4' anchorage. The fall occurred in the regular travel-way for the weekly airway examiner and was reported via hot line.

Employee stated that he was loading a 5 gallon hydraulic oil can on a pallet and he twisted his back. He said he felt a pop in his lower back. Employee went to the doctor over the weekend and was diagnosed with a pulled muscle. He notified the mine site on 9-1-15 that he would be on light duty for 1 week. Employee returned to regular duty on 9-8-15

Employee stated that he took a ride to #1 Belt Tail to spray off batteries. He repositioned himself and turned allowing his hand to contact the sequence roller on the belt line. A short bolt was welded on the sequence roller for the sensor to identify the rotation of the roller. He was struck on the knuckle of his outermost metacarpal requiring 6 sutures.

Employee was bolting 3 left and attempted to install a t-3 strap. He installed one side of the strap and attempted to push up the other end with the drill pot. The strap slipped off of the drill pot and sprung back striking him about 1" above his left eye. The employee received 8 stitches.

Employee stated that he parked the shuttle car in #5 right and exited the operators canopy to walk to the miner and assist with the move. After he exited the shuttle car he stepped on a rock and slipped and fell against the rib. He injured his left shoulder and neck.

Employee stated that he was cleaning his permissible ride and attempting to water the batteries. The ride has a spare tire attached to the battery lid cover and when he moved the spare tire, he felt a muscle strain in his back. The employee finished his shift but he went to the doctor on 3-15-15. He was placed on light duty for 1 week with a return to full duty on 3-23-15.

Employee received a laceration to the lower lip that required 5 sutures to close the wound. Employees states that he was tightening the bolts that secure the cutter motor to the frame of the cutter boom. Employee fell into the machine as he was pushing down on the ratchet handle.

Employee & foreman were training conveyor belt and employee lifted the structure to lengthen the chain that the structure was suspended from the roof with. Employee jerked up on the belt rail and then felt a pain in his lower back. employee was given restricted duty. Employee was working as a mine examiner.

Employee was struck on the top of the head by a chain hoist. Hoist was hooked to a belt hanger bracket and around a feeder conveyor chain that was fouled. The belt hanger failed while ratcheting the hoist resulting in the hoist striking the employee. Employee was transported to CAMC General Hospital, where he received two staples in his head and was then released.

A roof fall was found at 12:50 PM on 8/15/2014 of the Buckshot Mains of the Baker room section. Roof fell above anchorage from survey station 12599 to survey station 12600. This was in the # 6 and 7 entry intake. Stopping line was set over and intake was re-established.

Employee states that he was removing the miner cable from a cable hanger that was hung to the mine roof when the corner of the outby rib fell, striking the miner in the lower left side of his back. Employee continued to work but, then felt pain and was transported to the ER where he was X-rayed and was diagnosed with a bruise and was given restricted duty for 3 days.

Employee states that he was at the inby corner of the intersection in number 5 entry inby the feeder dump point when the corner of the rib rolled out and struck his right foot causing the steel toe of the boot to mash his toes causing the toe nail of the toe beside his little toe to loosen. The nail had to be removed.

Mine was notified on 7/17/2014 that employee had diagnosis of mild simple pneumoconiosis.

Employee states that he had installed the last roof bolt in the cut when a rock fell and struck the canopy of the bolter and broke into pieces. Employee states that he was struck on the shoulder by a rock that was about 1 inch thick by 7 inches long.

Electrician was removing the tape and splice jacket from the 208 continuous miner cable. As employee cut across the cable, the utility knife slipped out of the boot and this resulted in the employee receiving a laceration to his left hand. The laceration required 8 sutures to close.

Employee received a fracture to the end of two of his toes while assisting the roof bolter in backing up to install a cable anchor. Employee slipped on a flat rock causing his foot to slide under the edge of the right rear tire. He was released back to work and has worked his normal duties.

Mine was notified on 03/28/2014 that employee had diagnosis of pneumoconiosis.

Employee was using a cut off tool to cut a copper bar. Employee was holding the copper bar with a pair of channel locks and as he was cutting the bar the cut off tool kicked and struck the top of his finger with the rotating disc causing a laceration that required 3 sutures to close.

A roof fall was discovered in the Left Return of Byrd 2 Mains. The fall was in the #1 Entry between survey stations 1388 and 1372. The roof fall does not impede ventilation and is not a travel way. The area had been timbered off to travel.

Employee passed away from unknown cause. The time the employee passed away was an approximate time.

Employee was attempting to raise the flip up shield on the continuous miner and as he was lifting the shield his foot slipped causing him to let go of the shield in order to catch himself. This resulted in the shield coming down and mashing the employer's hand. The employee received a crack to the 3rd metacarpal bone of the hand.

Employee was found passed away from unknown causes. Investigation not complete. The time the employee passed away was approximately 1:20 pm.

EE states that as he was examining the regulator he observed that the roof needed scaled. As he scaled the roof a piece of roof that measured 15"X10"X2" thick, struck him on the top of his forearm. EE finished his shift and went to Med Express where an X-ray was taken and no injury was noted. EE continued to work and perform his regular duties, Please see attached page.

Employee was sitting in off-road coal truck while being loaded. After loader operator dropped load from bucket, the operator caught bucket on truck bed, pulling off-side of truck off the ground causing driver to strike his knee on steering wheel of truck.

Employee states that he was helping to move a water pump. After moving pump he felt a burning pain in his knee. Employee informed company on 11/12/2013 that he has been going to physical therapy.

Employee fell and in trying to catch himself he raked the end of his index finger down the end of the frame on the belt feeder. This action caused the nail of his finger to pull off. Employee received sutures when a protective foil covering was sutured into the nail bed to promote nail growth.

Employee states that he was assisting in moving the roof bolter out by the last open cross cut for the end of the shift. Employee states that he was reaching down to pick up the trailing cable and stepped in a rut in the roadway. This resulted in twisting his ankle.

Employee states that he had lifted a rub rail to install on the miner. After lifting employee states he felt tightness in his back. Employee states that he then went to remove the rub rail and felt pain in his back. Received documentation on 9/17/2013 that employee was diagnosed with a lumbar strain.

Employee states that he was riding in a rail mantrip that slid into the rear of a parked rail mantrip. Employee was transported to CAMC Hospital where he was treated and released and told to follow up with Strain Clinic. Strain Clinic removed employee from work.

Employee states that he observed another employee standing under a loose rock and alerted him to move. Employee states that he then started looking at the roof where he was standing and he looked up and a small rock dropped from the roof, striking him in the upper lip. Employee received 3 sutures to close the laceration. He returned on the next shift.

Employee states that he twisted his ankle after stepping into a rut in the roadway. Employee was seen at CAMC General and referred to Sprains & Strains Clinic. Employee was placed off of work on 8/14/2013. No rut could be found at the alleged incident site.

Employees states that he bent over and picked up the end of a 6 inch Aquamine pipe and then turned to walk and felt pain in his back. Employee did not report incident to supervisor. He reported the event on 8/2/2013 @ 8:21 PM on the call in line. On 8/6/2023 employee called and asked if he needed to come in and have an incident report filled out.

Employee was attempting to remove a water line coupling by inserting the sharp end of a pick hammer into a crack in the coupling and then striking the hammer with a claw hammer. While striking the pick hammer, a piece of the hammer head chipped off and imbedded into the side of the knuckle of his left index finger. The chip of metal had to be removed by a surgeon.

Employee was traveling down the East 3 Belt Scoop roadway and heard a coal rib pop and when he turned to look he was struck by coal on the left hand. He received a laceration to his ring finger that required 8 sutures to close.

Employee states that he pulled the ventilation curtain down to install a center line and was struck by a piece of rock that would weigh 35 lbs. The rock fell approximately 11 inches and struck employee on right shoulder. Employee had an xray c-scan and an mri and all were negative. Employee complains of numbness in right leg and difficulty in walking.

Employee states that he placed his hand on the conveyor boom and leaned over to get pull rope to hang cable on the hook on the miner boom. As he leaned over his stomach hit the tram lever and this caused the miner to move and pin his hand momentarily between the coal rib and the miner boom.

Employee and co-worker were installing track rails and had positioned a mine jack under the rail and had lifted the rail off the mine floor in order to install a rail tie. Employee asked the co-worker to use a bar and pry on the rail. When the rail moved the rail fell from the jack and struck employee on the foot. This resulted in a fracture to the foot.

Employee and co-worker were splicing a East mains #2 belt and in the coarse of cutting the belt, he received a laceration 2inches above his left knee. Employee received 6 sutures and was instructed to stay off work.

Employee received a laceration to his left forearm while helping bolt crew load their bolts onto roof bolter. The steel band that was binding the bolts together had been cut and 200 of the 250 bolts had been loaded. The steel band suddenly sprung and struck employee in the arm resulting in a laceration.

Employee alleges that he slipped and twisted his knee. Employee had previous non-work related surgery on his knee. Received Medical report on 12/18/2012 stating that an MRI was vague in deter mining injury to the knee but the MD thinks there is a tear of the meniscus and request surgery.

Employee states that he was climbing out of motor and stepped on draw rock causing his foot to slip and twisted his knee. Received medical documentation on 9/19/2012 that confirmed an injury to employee's knee had occurred.

Employee states that he struck his head on the underside of scoop canopy. Received medical documentation on 7/27/2012 of neck strain and physician gave employer two days off from work.

Employee stated he was pulling a timber out of a banded pile of timber and had to pull hard and when the timber suddenly became free and caught his finger between the timber and the scoop bucket. Employee was seen at local hospital & released to work with no restrictions. Received medical documentation on 5/11/2012 that his finger was fractured.

Received medical report on 4/13/2012 that an injury occurred to employee that stated that he had climbed upon a scoop to adjust the ampere setting on a bonder welder and fell to the mine floor as he was getting off of scoop. Employee stated that he fell on his back.

Employee was holding a wrench & another employee was striking the wrench. Employee that was holding wrench repositioned his hand and when the wrench was struck it pinned his finger against the frame of the scoop. This resulted in employee receiving 5 sutures to close the laceration.

A roof fall was observed at the enter section of survey station # 1588 in the # two entry of the left return of Byrd 2 Mains airway. Ventilation is not impaired. The # one entry is open for travel and ventilation. Area will be timbered off.

Employee was struck by drill wrench that was lodged between the drill boom and the tool tray. As employee lowered the drill boom the drill wrench struck employee on his index and middle finger. Employee received a laceration to the index finger and middle finger that required 4 sutures to close. He was treated and released to work.

Occupational illness. Employee was in # 5 entry and felt pain in knee. Employee has a history of problems with his knee. Receive report of treatment and MRI reveled Grade 2 patellofemoral chondromalacia

Employee was walking in # 5 entry and felt pain under his left patella. Employee stated that he had knee problems previously. Company received medical documentation that made the injury a reportable. Employee had a tear of the meniscus of the left knee.

Employee installed a fly curtain and was cutting the curtain with a utility knife. He was holding the knife with his left hand and the curtain with his right and in cutting the curtain he cut through his glove into his index finger.He received 5 sutures and returned to work on his next scheduled shift.

A roof fall was found in # 4 entry,1 cross cut inby survey station #1902 of Bays Mains. Roof fall does not impede ventilation and is not a travel way.

Employee was loading an empty wooden pallet onto an empty rail supply car and he felt a sharp pain in his arm and shoulder. Employee continued to work until 7/28/2011. Company received medical documentation on 9/28/2011 that incident was reportable.

Employee was walking around shuttle car and stepped on a rock, causing him to stumble and twist his knee. After the incident, the employee continued to work until June 16, 2011 when his physician removed him from work.

Employee was attempting at install a roof bolt, roof bolt plate & a roof strap. As he shoved the bolt up in the hole, the bolt bent causing the plate to apply pressure on the roof strap. Employee had his hand guiding the strap into position and the strap pinched the ends of his middle and ring finger.

EE was hanging curtain in miner entry near the end of his shift when he walked through a curtain already hung and hit his head on roof bolt.

Employee twisted his knee when he fell over loose coal making a face examination with a dimmed mine light. Employee stated that he had knee problems in the past. Employee had arthroscopy on his knee and the medical report received on 12/02/2010 diagnosised that the employee had grade IV chondromalacia.

Employee received a fracture to the right wrist while operating a permissible personnel carrier. The steering wheel spun in a backwards motion and employee was struck in the wrist with the steering wheel knob.

Employee was attempting to install a roof bolt and was in the process of starting the drill hole. Employee was holding the drill stell and started the rotation while holding the drill steel. Employee's glove caused a laceration to the index and ring finger. Sutures we required on both fingers to close the lacerations. Employee was released to return to work.

Employee was handling 50 lb bags of rock dust and then felt pain in his lower back. Employee finished the shift. Employee went to a physician the next day & the physician removed employee from work.

Employee was attempting to install a cat idler on the operator side of continuous miner. Employee placed his feet against the idler & used his legs to push the idler in place, Employeee finished shift & reported incident on 6/07/2010. Company received medical documentation on 6/28/2010 of lower back strain

Employee was drilling a hole for a roof bolt installation when a section of roof around the hole loosened and came down on the drill steel, hitting the drill pot & then hitting employee on the knee. Employee sustained a contusion to the knee. Doctor restricted employee from working on 5/21/2010.

Employee was walking when he slipped and fell on slick mine bottom. Employee worked until 5/06/2010 when he was taken off work by physician.

Employee was carrying jacks & timbers to timber off a roof fall. While carrying a heintzmen roof jack, employee's foot slipped causing the employee to twist his knee. Employee had knee arthroscopy on 8/25/2010. Company received medical evidence that an injury occurred and determined the incident as reportable.

Employee stated that as he was moving miner cable out of roadway that he felt a pop in his back & felt pain. Diagnosed as a strain. Company notified on April 1,2010.

Employee & coworker were attempting to move a set of wooden steps in order to scale some loose roof. While lifting the steps, employee stated that his bicep popped & he immediately felt pain.

Employee was using a crib block to bump a motor in place. The old motor had not been removed from the work area and as employee swung the crib block back,he struck the old motor with his left hand. This injury resulated in a laceration & slight fracture of the ring finger of his left hand. One suture was required to close wound

Mine Examiner found a roof fall in the left return off of Byrd Mains # two belt at # 50 cross cut between # 1 & # 2 entry. Left of survey point # 1556. Air movement is not impaired & no employees were present in the area.

Employee slipped on the muddy mine floor while setting a mine pump. Employee was twisting while moving a pump. This resulted in an injury to the right knee. Employee worked until 8/12/2009 when surgery was done to repair knee. Employee returned to work on 8/21/2009.

Employee was on his knees attempting to square conveyor belting by cutting it with a utility knife. He was pulling the knife toward himself when it cut across the belt and into his right leg just above his knee. The laceration required seven sutures to close.

Mine examiner was making an examination of the Byrd Mains left return & observed an unplanned roof fall at an intersection 1 cross cut inby survey station #1346. Travel or ventilation in this area has not been compromised.

Employee was repositioning the continuous miner & while observing the movement of the continuous miner employee was walking backwards away from the machine, tripped & fell to the mine floor. In falling to the mine floor the employee hyper-extended his knee.

Employee fell to mine floor due to slick mine floor. Received an abrasion and a contusion. He worked his next scheduled shift and reported his leg as swollen and hurting. He was instructed to seek medical attention and was hospitalized with an infected leg.

While examining a de-watering pump 3 cross cuts inby syrvey station 213 the examiner found a roof fall at an intersection 1 cross cut inby survey station 213. Heintzmann's were set at each openings to the area. Fall was in an intake air-way. Travel and escapeway have not been compromised in the area.

Employee was making a cable pull hook for the boom of 004 Miner. The 1 1/2 inch rolled steel had been heated & a crib block against the mine rib & using the swing action of the conveyor boom of the miner to achieve the desired bend in the hook, the crib block struck employee in the thigh of his leg.

Employee was removing insulation from miner trailing cable and utility knife slipped causing a laceration to left thumb. Employee finished shift and was seen at Raleigh General ER where thumb was sutured to close the wound. Employee returned to work on 12/17/2008 and has not last any work days from work.

Employee was carring two bags of sand to replenish the sand to a supply motor. Twisted his right knee while rotating his body. Company was made aware of scheduled surgery on December 28,2008

Employee was retrieving a tool from the back of a Johnson Indutries personnel carrier and was struck by a combination of rib coal and roof rock. The material hit employee in the lower back and his legs, forcing him into the back of the personnel carrier. He sustained multiple fractures to the lower leg, crushing injuries to the right pelvis and right ankle.

Employee & co-worker we tighting a bolt in a flex board hold down when co-worker's wrench slipped off and employee was hit in the mouth with the wrench. This caused a front tooth to be chipped. Employee has not had any lost days & had tooth repaired 10/22/2008.

Employee was removing a nylon cable hanger from a miner power cable and water line. While removing fron a roof bolt the cable got tension on it and as the hanger was removed, the s-hook fastener cut employee's nose.

Employee was in the process of making a belt splice. He struck the belt splice template knocker a glacing blow with a hammer & struck middle finger between the knocker & the hammer handle. The employee finished the shift and then sought medical attention. Employee received 5 sutures to close the laceration. Employee had removed wet gloves in order to grip the hammer handle better.

Employee was removing roof bolts from scoop bucket and received a cut from the metal band that binds the roof bolts for transportation. Employee received 4 sutures and has not lost any work days.

Miner was fueling the plant dozer and the fuel hose was wrapped around his leg. When he finished he dropped the hose, he than lost his balance and fell from the rear platform on the dozer. EE pulled something in his shoulder and is awaiting diagnosis.

Employee was skiving belt and was pulling knife with right hand toward his body and the knife slipped and then the knife stabbed the palm of his left hand. Cut required two stitches to close.
