Employee states that they were struck in the leg and foot by an 8 inch I beam that came loose for a belt take-up that was being removed as part of a section move. Employee suffered a fractured ankle and fractured metatarsal bones.

Employee states that EE received an injury to EE's hand while raising the lid on the scoop batteries. Employee states that the support rod for the battery lid did not latch causing the lid to strike EE's hand.

Employee stated was operating the Joy 14/15 C/M machine on the Unit 1 Section. In the process of operating the machine cleaning up some gob a rock flew off of the C/M head and struck the employee in the mouth. This caused EE's tooth to be chipped. NOTE: This employee didn't seek medical treatment until 4/8/2019 and that is why it is now being reported.

The rock measured approximately 2.5' in length X 1.5' in width X 1' in thickness. NOTE: This employee didn't start missing work until 4/2/2019.

A piece of rock fell from the mine roof/rib and struck EE on the right shoulder causing EE to fall with the rock landing on EE's right leg. Employee was diagnosed with a fracture to the tip of EE's right shoulder clavicle bone which resulted in restricted days.

Employee was walking to check a pump in the return when they slipped on a slate rock, while trying not to fall they twisted the left knee and heard a popping noise. Employee finished shift and returned to work with no restrictions for next schedule shift. On 6/1/2018 employee had knee surgery and began missing work at which time this incident became reportable.

Employee stated there was a rock fouled underneath the chain of a shuttle car. In order to remove it employee was striking the pin out that holds the flight in place. In doing so the flight broke free and struck employee in the face causing a laceration that required several stitches.

Injured employee stated ee and some other employees were attempting to remove a cutter motor using a chain and scoop. In the process of pulling a bolt broke off the cutter motor and struck the injured employee in the abdomen causing a contusion. Employee was given modified duty. At 10:10am Airway Examiner discovered a roof fall in #6 entry on Byrd Mains #2. The fall fell above the ancourage point and measured 45'x19.5'x5' thick. The fall was dangered off and lifeline was re-routed to #7 entry to provide a continuous means of egress from section. Area was previously mined 8 years ago and 4' fully grouted bolts utilized.

Employee was dismounting a scoop. When EE was standing EE's foot went in a hole causing employee to fall forward twisting EE's ankle. EE reported to work on EE's next scheduled shift on restricted duty.

A roof fall was discovered on 6-28-2016 located at Buckshot #1 break 29 Spad 6763 in the #6 entry. The fall measured approx 70' long X 19' Wide X 5' high. The area was dangered off and all approaches breakered off. IT will not be cleaned up. The area was originally mined in January 2012.

Employee stated - noticed a rib roll had dislodged a timber. The timber was 6-1/2' tall and EE stood the timber back up to set into place. While lifting EE felt a sharp persistent pain in left groin. Employee went to the doctor who suggested stretching and exercise. EE was released to restricted duty and returned to next scheduled shift.

Employee stated that EE noticed a tightness in arm at the end of shift. EE was unable to identify an event during the shift that would result in an injury. EE continued to work without missing a shift. EE later went to the doctor and needed surgery. EE worked up until surgery on May 25, 2016 and returned to work on light duty.

EE was utilizing a 4" cordless Dewalt angle grinder to remove a damaged lock from a hasp. Was holding the hasp with a pair of channel locks while cutting the lock. Grinder fowled and kicked back cutting through leather glove causing a laceration to left thumb. Laceration required 6 stitches and the ee was placed on light duty to keep the cut clean.

The employee was using a ratchet to pull in a conveyor chain on a shuttle car. The Chain fouled, so the employee attempted to release the chain ratchet. When EE reversed the ratchet, the handle turned loose and the employee suddenly stepped back twisting ankle. EE has been allowed to work light duty.

Mine lost power the morning of the incident. The Motor crew was attempting to come outside and was unlatching elec door to move motor through the door. One jack was unhooked while the second door was being unlatched. The mine fan power was restored and the air current slammed the door shut striking the injured miners arm. EE has been allowed to work light duty.

The Buckshot crew left the section on Company #714 track mounted mantrip. One of the section employees placed a used piece of drill steel on top of the mantrip. When the mantrip pulled through the section airlock doors, the steel caught the door and spun around striking the injured employee.

Employee traveled to #6 Right of West Mains to check #6 pump. EE traveled through a mandoor and crouched down due to the low cavity height. The walkway had ruts from equipment EE stepped on the peak of a rut spraining EE's right knee. EE worked regular duties/shift and had surgery to repair meniscus on 8/18/2016. EE returned on EE's next scheduled shift on restricted duty.

An unplanned roof fall was discovered near Spad #11463 at the mouth of the West Mains 6 left bleeder panel. The fall measured approximately 20' long and 40' wide and fell just above the 4' anchorage. The fall occurred in the regular travel-way for the weekly airway examiner and was reported via hot line.

Employee stated that he was loading a 5 gallon hydraulic oil can on a pallet and he twisted his back. He said he felt a pop in his lower back. Employee went to the doctor over the weekend and was diagnosed with a pulled muscle. He notified the mine site on 9-1-15 that he would be on light duty for 1 week. Employee returned to regular duty on 9-8-15

Employee stated that he took a ride to #1 Belt Tail to spray off batteries. He repositioned himself and turned allowing his hand to contact the sequence roller on the belt line. A short bolt was welded on the sequence roller for the sensor to identify the rotation of the roller. He was struck on the knuckle of his outermost metacarpal requiring 6 sutures.

Employee was bolting 3 left and attempted to install a t-3 strap. He installed one side of the strap and attempted to push up the other end with the drill pot. The strap slipped off of the drill pot and sprung back striking him about 1" above his left eye. The employee received 8 stitches.

Employee stated that he parked the shuttle car in #5 right and exited the operators canopy to walk to the miner and assist with the move. After he exited the shuttle car he stepped on a rock and slipped and fell against the rib. He injured his left shoulder and neck.

Employee stated that he was cleaning his permissible ride and attempting to water the batteries. The ride has a spare tire attached to the battery lid cover and when he moved the spare tire, he felt a muscle strain in his back. The employee finished his shift but he went to the doctor on 3-15-15. He was placed on light duty for 1 week with a return to full duty on 3-23-15.

While striking the pick hammer, a piece of the hammer head chipped off and imbedded into the side of the knuckle of his left index finger. The chip of metal had to be removed by a surgeon.

Employee fell to mine floor due to slick mine floor. Received an abrasion and a contusion. He worked his next scheduled shift and reported his leg as swollen and hurting. He was instructed to seek medical attention and was hospitalized with an infected leg.

While examining a de-watering pump 3 cross cuts inby syrvey station 213 the examiner found a roof fall at an intersection 1 cross cut inby survey station 213. Heintzmann's were set at each openings to the area. Fall was in an intake air-way. Travel and escapeway have not been compromised in the area.

Employee was making a cable pull hook for the boom of 004 Miner. The 1 1/2 inch rolled steel had been heated & a crib block against the mine rib & using the swing action of the conveyor boom of the miner to achieve the desired bend in the hook, the crib block struck employee in the thigh of his leg.

Employee was removing insulation from miner trailing cable and utility knife slipped causing a laceration to left thumb. Employee finished shift and was seen at Raleigh General ER where thumb was sutured to close the wound. Employee returned to work on 12/17/2008 and has not last any work days from work.

Employee was retrieving a tool from the back of a Johnson Indutries personnel carrier and was struck by a combination of rib coal and roof rock. The material hit employee in the lower back and his legs, forcing him into the back of the personnel carrier. He sustained multiple fractures to the lower leg, crushing injuries to the right pelvis and right ankle.

Employee & co-worker we tighting a bolt in a flex board hold down when co-worker's wrench slipped off and employee was hit in the mouth with the wrench. This caused a front tooth to be chipped. Employee has not had any lost days & had tooth repaired 10/22/2008.

Employee was removing a nylon cable hanger from a miner power cable and water line. While removing fron a roof bolt the cable got tension on it and as the hanger was removed, the s-hook fastener cut employee's nose.

Employee was in the process of making a belt splice. He struck the belt splice template knocker a glacing blow with a hammer & struck middle finger between the knocker & the hammer handle. The employee finished the shift and then sought medical attention. Employee received 5 sutures to close the laceration. Employee had removed wet gloves in order to grip the hammer handle better.

Employee was removing roof bolts from scoop bucket and received a cut from the metal band that binds the roof bolts for transportation. Employee received 4 sutures and has not lost any work days.

Miner was fueling the plant dozer and the fuel hose was wrapped around his leg. When he finished he dropped the hose, he than lost his balance and fell from the rear platform on the dozer. EE pulled something in his shoulder and is awaiting diagnosis.
