FATAL VICTIM AND TWO OTHER MEN REACHED DOWN TO PICK UP MINER CABLE WHEN A PORTION OF COAL RIB AND CAP ROCK FELL SUDDENLY CRUSHING AND PINNING THE VICTIM.
