INSTALLING ROOF BOLTS ON SECTION.INSERTING RESIN AND BOLT INTO HOLE WHEN ROOF ROCK FELL STRIKING HIM ON TOP AND BACK PUSHING HIM INTO BOLTING MACHINE.THIS CAUSED INTERNAL CRUSHING INJURIES.

VICTIM WAS INSTALLING BELT HANGERS AS PART OF REHABILITATION PROJECT. HE HAD INSTALLED HANGER & ASSISTED COWORKER. VICTIM RETURNED TO LEFT SIDE OF BOLTER HEAD NEAR DRILL CONTROLS. WHILE HE WAS WAITING HE WS STRUCK BY MASSIVE RIB ROLL WHICH OCCURRED SUDDENLY & WITHOUT WARNING.
