Employee was operating a loader when bit by spider in right ankle

EE was stung by a bee

Employee was completing paperwork near EE's toolbox in Bay 4 of the Truck Shop when a gust of wind blew dust/debris into EE's right eye. The employee was wearing safety glasses at the time of the incident.

The employee was performing normal job duties at the S Tank-manifold when they was bitten/stung by an unknown insect. Employee went to ER and was perscribed Benadryl and Prednisone.

A customer truck driver came to pick up a load of bulk cement when EE experienced a personal health related issue.

At 8:35 AM, Employee radioed that they were having difficulty breathing. I met them at the ready line, where they reported chest pain and a migraine. They initially declined medical assistance but later requested it due to concerns about their heart and family history. I immediately called 911, notified Safety and my supervisor, and transported EE to the front gate to meet EMS.

IE twisted an ankle while assisting a co-worker who had been knocked to the ground by the disturbance of rib sloughage material. Injury was not the result of a training or compliance issue.

During shift change, the operator sustained a back strain while reaching behind the seat to retrieve EE's lunch bag. The discomfort was reported immediately following the movement.

Operator was doing a walk around on their haul truck and got dust/debris in their eye. Employee lost time on 08/14/2026.

Employee squatted down cleaning accumulations, when they stood up left knee popped. Unable to bear weight on left knee.

The IP was on the surface retrieving parts for a piping project when they were stung by a bee. The bee stung them on the R Side of their neck and experienced localized swelling. They had no other symptoms. Using the IP reported to the clinic where they were given a OTC antihistamine. The IP returned directly to work.

EE came into the garage and stated EE didn't feel well and asked for someone to take EE home. An EE called EE's supervisor and notified them that EE didn't look or feel well. proceeded to take EE to Lehigh Valley hospital in Hazleton. EE was subsequently life flighted to Lehigh Valley Wilkes-Barre and received emergency heart surgery (2 stents) and is currently in recovery

When getting into the operator's compartment (kitchen) of the roof bolter they twisted and felt a pinch in their back that radiated down their leg. Employee is off and awaiting results from the MRI and doctor visit.

Employee was walking in a work area and suddenly felt like EE was being bit. The employee removed EE's shirt and had other employees examine EE for bites. The employee described feeling stinging. The employee did not report the incident until 7/28/2026 by way of texting after not showing up for work.

Employee was replacing a blown hose on the angle bolter. When they were getting down from the bolter, they placed their weight on to left leg and left knee gave out. Employee finished their shift. Was seen on the 22nd of July and diagnosed and taken off work with a possible tear to their ACL in their left knee.

On the morning of July 21, 2026 at 11:01 the IP was found unresponsive on the deck of DDS-09. Emergency response team was called, IP was transported to hospital for evaluation

Employee was performing routine work and bent over to connect strongback. As employee stood up, they felt a sharp pain in their lower back. Event went reportable 07/22/2026.

EE was weed eating, hit a bees nest, and received stings on their right thigh. EE went to clinic, had 4 stingers pulled out, and received over the counter meds. EE had a follow up the next day where it was found that their stings had become infected. They were given a prescription medication. EE has remained full duty.

Employee was finishing washing out tail pulley with a hose and went to get back into skid steer. When employee swung leg into the equipment, felt a pop in RH side groin area accompanied with severe pain and numbness of the thigh. Employee reached out on radio and returned to shop. Employee was seen by Fit for Work who recommended medical follow-up - suspected hernia.

While walking to the dumpster, employee felt that they stepped wrong and twisted the right knee. No trip, fall, contact with an object, vehicle, or equipment, or hazardous surface condition was reported. Employee resigned 7/22/2026 to start a new job before completing follow-up medical evaluation or receiving a full-duty release.

Operator exited their equipment and began walking to the blue room, while walking a gust of wind picked up some dust and blew it in the operator's direction, resulting in dust in the eye.

TM was walking and felt pop in knee. This injury was initially considered first aid. Went to clinic on 7/29/26.

Subject completed shift and was getting into their vehicle when they were stung by a wasp. Subject is allergic so they administered an EpiPen, drove home, and had their spouse take them to the ER. While at the ER, employee was given Benedryl. No further actions were taken.

IE was walking in battery barn when IE felt pain in their back. IE sat down to try and eat lunch but the pain intensified. IE was transported to Hospital by a company official.

Team Member was stung on the right hand while retrieving a ladder from storage.

Operator of Haul Truck was attempting to mount truck and strained back.

MSHA Inspector was exposed too feed dust during conversation with Cemex employee, MSHA inspector stopped Cemex from preforming their job duties.

Employee was hooking up input and output lines to a gear pump. Employee started feeling dizzy and briefly lost consciousness. EE was taken to the clinic for evaluation and treated for a personal medical issue.

Miner squatted down to seal a manway cover on a railcar and felt something pinch in their lower back area causing pain that increased over time.

A belt examiner was found alone in the mine and unresponsive. Attempts to resuscitate the employee were unsuccessful and the employee was pronounced dead upon arrival on the surface. It's our position this death was not mine related and we reserve the right to retract this filing upon MSHA chargeable ruling.

EE was leaving dry and they high fived another EE. Reported immediate hand pain. Seen at clinic 6/18, did not file a C4. On 6/25 EE went to Hospital and filed a claim for workers' comp. Referred to ortho who did previous surgery. X-rays shows issues with screws and plates from previous surgery. Both providers noted this issue is non-occupational.

Employee was walking up entry and felt a pain and burning in left knee , EE advised pain increased with any movement , also advised there was no mechanism of injury. Employee was brought to surface and transported to medical facility for treatment

Employee was inspecting an oil leak under a loader. They strained their right side while climbing out to their feet.

Employee was entering a silo by way of a work platform and as EE entered the silo EE heard a pop in EE's left knee.

Our coworker experienced a medical emergency while getting into EE's personal vehicle. EE later succumbed at the Lehigh Valley Cedar Crest Hospital.

Employee was standing in the shop and went to move when EE felt a pop and experienced pain in EE's lower left leg, ankle and calf area.

Patient states EE was bending over with a caulking gun to patch a hole, when EE "tweaked" EE's lower back. Strain with RICE method and OTC medications.

Employee went to the main office complaining of head ache. EE took some aspirin. Ate some pizza rolls and sat down to rest. Office manager noticed a sound coming from the employee and noticed that EE was unresponsive.

EMPLOYEE WAS STEPPING DOWN INTO THE MINI X 303.5 TO BE SEATED. WHEN EE STEPPED DOWN ALL OF EE'S WEIGHT WAS ON EE'S LEFT HIP AND IT POPPED CAUSING SEVERE PAIN. EMPLOYEE WAS ISSUED A PRESCRIPTION MEDICATION MAKING THIS REPORTABLE. EE WAS NEVER PUT ON LIGHT DUTY.

Employee was starting the shift and was participating in a stretch and flex program when EE felt a pop in EE's lower back leading to pain and discomfort.

The operator was accessing the cab of a CAT 633D Scraper. With the door open, the operator began to enter the cab when a gust of wind caused the cab door to close. The door impacted the operators back. More than 2 hours later, the operator reported EE felt some pain in EE's groin region.

Employee was going down to belt deck around 8 belt rounded the corner baring all weight on right leg, turned the corner like individual usually does and felt a pop or snap, couldn't put weight on it after. Waiting evaluation Doctor Recomend Surgery June 06 2026

Somewhere between 12 and 1230, an EE asked EE to clean up the catwalk on the screen deck.  EE was performing EE's normal work at a normal pace. While other EE was tramming the excavator, they noticed EE in distress and responded. EE went into cardiac arrest, 911 was called, CPR started, but EE did not recover.

Personal Health Issue not due to any chemical or physical factor from the mine. Only one person affected by this personal medical condition.

Employee was kneeling to tighten bolts down on #2 dust valve (Calcination), upon standing EE felt something pop in EE's right knee.

Employee was exiting a man van and as they put weight onto their right leg, they felt a pop in their right knee. The event went reportable 07/28/2026.

An employee was in the Raw Mill attempting to remove reject stone, when a raccoon ran under the skid steer they were operating. The employee stopped the skid steer and the raccoon ran into the corner and got trapped under a beam. The employee attempted to rescue the raccoon, but was bit in their right index finger. Rabies exposure protocol was followed at the emergency room.

Employee picked up sample bag and tossed it into the bed of the light vehicle. When turning to get into the light vehicle felt a sharp pain pain in low back down to right hip.

The employee bent forward and subsequently experienced pain and numbness in the lower back and legs.
