EMPLOYEE WAS STEPPING DOWN INTO THE MINI X 303.5 TO BE SEATED. WHEN EE STEPPED DOWN ALL OF EE'S WEIGHT WAS ON EE'S LEFT HIP AND IT POPPED CAUSING SEVERE PAIN. EMPLOYEE WAS ISSUED A PRESCRIPTION MEDICATION MAKING THIS REPORTABLE. EE WAS NEVER PUT ON LIGHT DUTY.
Employee was starting the shift and was participating in a stretch and flex program when EE felt a pop in EE's lower back leading to pain and discomfort.
The operator was accessing the cab of a CAT 633D Scraper. With the door open, the operator began to enter the cab when a gust of wind caused the cab door to close. The door impacted the operators back. More than 2 hours later, the operator reported EE felt some pain in EE's groin region.
Personal Health Issue not due to any chemical or physical factor from the mine. Only one person affected by this personal medical condition.
Somewhere between 12 and 1230, an EE asked EE to clean up the catwalk on the screen deck. EE was performing EE's normal work at a normal pace. While other EE was tramming the excavator, they noticed EE in distress and responded. EE went into cardiac arrest, 911 was called, CPR started, but EE did not recover.
Employee was kneeling to tighten bolts down on #2 dust valve (Calcination), upon standing EE felt something pop in EE's right knee.
An employee was in the Raw Mill attempting to remove reject stone, when a raccoon ran under the skid steer they were operating. The employee stopped the skid steer and the raccoon ran into the corner and got trapped under a beam. The employee attempted to rescue the raccoon, but was bit in their right index finger. Rabies exposure protocol was followed at the emergency room.
Employee picked up sample bag and tossed it into the bed of the light vehicle. When turning to get into the light vehicle felt a sharp pain pain in low back down to right hip.
The employee bent forward and subsequently experienced pain and numbness in the lower back and legs.
An employee was inspecting a vehicle at the main gate when they were stung/bite by an insect on the right ear.
IE was bitten by some sort of spider/insect, and EE's right leg became infected. IE reported it on April 24th and did not miss work until 04/29/2026.
Employee found slumped over in haul truck while waiting to load. Unresponsive to radio. No prior symptoms reported. Transported for medical care; later diagnosed with suspected stroke and brain clot. No known work-related cause identified.
Employee experienced a seizure while making observations on the computer in the AA Lab room of the Merrill Crowe Facility. Another employee accompanying them noticed immediately and caught them bringing them safely to the floor where they laid EE on their side. EE just returned to work from medical leave from the same condition in November of 2025.
While conducting tailings dam safety inspections, the wind increased, causing increasingly dusty conditions, resulting in dust getting into the employee's eye.
Employee was walking across the feeder and felt a pop in EE's right knee. EE was seen by an orthopedic specialist on 4/23/26 and was given a full release with no restrictions. EE continued to work until 5/1/26 when EE was seen by another physician who gave restrictions preventing EE from performing EE's normal job.
Employee was working in the lab when EE began feeling ill. EE was transported to the local emergency room by the site foreman, where EE was diagnosed with kidney stones. The condition was determined to be non-work-related
Mill Mechanic was moving a sensor into position for run operations and in the process pushed themselves up to move along the ring gear and felt a pop in their left shoulder.
Loader operator was dismounting. placed right leg on the ground and went to turn to the right. Right foot did not move resulting in the force going to the right knee. Doctors initial ruling is a sprain
Employee experienced a medical emergency while at the mine site. The employee then fell to the floor from a leaning position. The employee was transported via county EMS for further care.
The Injured Person (IP) was cleaning out the Roto-packer and bent over to cut a partially empty 50 lbs bag to discard remaining contents, and as the IP bent over, EE felt a pop in EE's lower back (left), and the IP had to take a minute before standing up straight. IP reported the incident to the Lead Operator. IP was taken to Urgent Care.
Employee had not returned phone calls for approximately 2 days so a well being check was called into the County Sheriffs Office as well as to a trusted member of the community. The community member found employee on the ground in a shack adjacent to employee's living area deceased from an apparent heart attack.
The employee was found unresponsive lying next to a light plant that was not running. The time of the event occurring has an approximate window of 30 minutes prior to discovery.
Employee was walking down stairs. Felt a pop in their left knee.
Resubmitting new report due to glitch from previous report (E-Doc #3673211). An altercation between two employees. One employee was struck in the face multiple times and received closed fractures to the facial bones
Employee was lifting and turning their leg when it broke. This is the employee that had previously broke their leg at their house in August 2025 and it's the same exact leg. It fractured in the same location of a previously installed screw.
Team member was tightening a set screw in a slightly bent position, stood up, and experienced back discomfort.
Mechanic reported being bitten by a wasp on EE's left wrist during a hooper area inspection. The following day, the employee presented with redness and swelling. Medical attention was sought, and antibiotics were prescribed.
ERT training employee was acting as an unconscious patient. During a planned patient move another rescuer stepped on the employee's pinky finger.
Employee wasn't feeling well went to car and tried to leave other employee stopped EE to check on EE. figured out EE was having a heart attack pulled EE out Checked Vitals found no pulse and started CPR on EE. Employee called 911 and MSHA. EMS Arrived on location. and took EE to the hospital.
Operator on lunch saw EE's truck on pile of rocks. When contact was made, EE unresponsive, truck running, doors locked. Supervisor broke window to get to EE, EE remained unresponsive. CPR started and continued while EE transported to Hospital. This appears to be a possible cardiac event. Investigation ongoing.
An employee experienced a medical emergency underground, and the cause was not immediately known. As a courtesy, MSHA's emergency line was notified. Following a thorough investigation by MSHA, the State of Illinois, mine and contractor management, along with a diagnosis from a licensed healthcare professional it was determined the event was not caused by a hazardous mine condition.
Employee was leaving shack to move the track mobile off the tracks. The wind picked up and blew a large amount of lime in their face, getting into their eyes
Employee was accessing the second deck of a Diester screen to check screens and their knee dislocated while getting in the screen. After imaging it was determined they had torn ligaments. Employee is now scheduled for surgery on 4/10/26
Employee twisted while changing hydraulic filters and began to feel a pain in their knee. later they asked to be checked out and was seen at the ER. An ORTHO appointment on 03/26/2026 revealed a torn ACL and they were given a restriction.
Pre-existing condition stemming from heart problems and malfunctioning defibrillator. While operating a water truck, miner experienced chest pain and dizziness and passed out after exiting the truck and asking for help.
Employee was walking to EE's work location and felt something in EE's right eye. Wind and dust could have been a factor.
According to the EE, EE "just turned and knee gave out". EE stated that this was a "preexisting injury that EE thought was healed". EE finished EE's shift and did not seek medical attention until 3/12/2026. EE was later caught walking through the location while on EE's cell phone and EE was walking without issue.
EE downloading data off monitor, disconnected monitor from computer, twisted left to grab next monitor, felt sharp pain in neck, then stretched their neck and heard popping. Seen at clinic, diagnosed with cervical strain, released with restrictions. Site offering modified duty.
An unresponsive truck driver was found in the driver seat of their haul truck. 9-1-1 was notified at 12:51P on Friday, March 6, 2026.
Squatting to retrieve a rope and caused a back spasm.
IP woke up, stepping out of bed experienced pain in right leg, mainly on right knee and foot.
welding bit block, curtain around worksite, when they rolled the drum the curtain blew in wind, employee felt like EE had dust in eyes.
Employee was switching seats to go back to the feeder, twisted right knee and said they heard a loud popping sound.
Miner was walking down the stairs from a dry powder bagging machine platform and hyperextended right knee while stepping down causing pain.
On 2-19-26, a teammate reported eye pain in BD 10. After using an eye wash, they requested a medical evaluation on 2-23-26 for persistent discomfort. While no debris was found, a corneal abrasion was diagnosed, and prescription drops were issued for recovery.
Employee was using a shovel to clear fines. The employee was found unresponsive, sitting with back against a wheel of the crusher. There were no physical signs of possible causes nor were there any witnesses. Employee is still being evaluated and tested for medical concerns.
EE was in the process of repairing the hydrated lime blowers when the wind blew dust into EE's right eye. Safety glasses were worn during the completion of the task.
Unknown, Death
The employee reported feeling unwell while operating a front-end loader. EE was transported to urgent care and subsequently referred to the local emergency department, where EE was evaluated and released without a definitive diagnosis. Additional medical testing is ongoing. At this time, the incident is not related to any work activity or workplace condition.
