AFTER REMOVING THE GUARDS AND LEAVING THE CONVEYOR BELT RUNNING DECEASED WAS PERFORMING CLEAN-UP DUTIES AT A TRANSFER POINT WHEN HIS CLOTHING BECAME ENTANGLED IN THE HEAD PULLEY SHAFT CAUSING HIS BODY TO BE ROTATED ABOUT THE SHAFT.
