55015D SUPT WAS IN ROUTE TO THE SECTION LOADING POINT EE WAS RIDING THE BUCKET END OF THE SCOOP MATERIAL FROM THE ROOF FELL FROM BETWEEN TWO BOLTS 41 INCH APART STRIKING THE EE ON THE CHEST IT 06643D APPEAR THE SCOOP STRUCK ROOF BOLT CAUSINGROCK TO FALL.
