FATALLY INJURED PERSON WAS STANDING ON HEAD OF CONTINUOUS MINER CUTTING A CROSS BAR TO LENGTH WHEN BROW AT EDGE OF ROOF FALL FELL STRICKING HIM A FEW SECONDS LATER THE RIB CAME IN THIS AREA WA S SUPPORTED WITH ROOF BOLTS BUT AREA WHICH FELL WAS PART OF A FAULT ZONE IN THE CENTER OF THE ENTRY
