Customer truck driver pulled on site and got load. Complained at scale that was having difficulty breathing. Driver stated was having back pain occur prior to arriving at mine site. This ended up being a medical condition of a blood clot. Driver is not a miner or employed by mine site. Blood clot not a result from any mining activity.

Employee alleges he was running on a flat surface toward the screen chute because it was plugged and his knee gave out on him. Employee had surgery on 12-08-15 and is losing time now.

Employee alleges he was cleaning off guard and some dirt and rocks came off of guard and hit him in his face. I just received Dr. notes on 01/08/2013 stating removal of foreign object in his right eye.

THE 2 FT PRY BAR HE HAD IN HIS LEFT HAND INADVERTANTLY CAME IN CONTAC T WITH THE PINCH POINT OF THE HEAD PULLEY DRAWING IN THE BAR AND HIS HAND. HE PULLED OUT HIS HAND BUT RECEIVED 2 BROKEN FINGERS.
