Employee was stringing chains over an I beam. One chain was pulled up and struck the employee on the right side of face.

Employee was working in a sump area near a sixty foot face. The crew was in the process of removing an old deenergized sump pump power cord when the new power cord, which ran along the face, shifted, causing loose rock to fall. As a result, the employee was struck on the back, causing abrasions and swelling. Employee is currently on pain medication and restricted duty

EMPLOYEE WAS REACHING FOR A CROSS BRACE FOR THE SCAFFOLDING SYSTEM AND AS HE BENT OVER TO PICK IT UP HE STRAINED HIS BACK.

EE WAS CLEANING MATERIAL OFF OF A CONVEYOR BELT. EE STATES THIS ACTION CAUSED AN UMBILICAL HERNIE. UNFORTUNATELY HE NEVER REPORTED THE INJURY TO HIS SUPERINTENDANT. THE COMPANY DID NOT LEARN O F THIS INJURY UNTIL THE EE WAS IN SURGERY AND THE HOSPITAL CALLED TO CONFIRM THE WORK COMP CASE. THE INCIDENT WAS UNDER INVERSTIGATION SINCE THEN BY OUR INSURANCE COMPANY. EE WAS LIGHTING A LP BURNER THAT IS INSTALLED THRU THE BOTTOM OF A SCREEN DECK (FOR DRYING MUD CAKED ON THE SCREENS). THE TORCH FAILED TO LIGHT, SO THE EE WENT TO GET A NEW STRIKER, BELIEVING HE WAS ALLOWING ENOUGH TIME FOR GAS TO DISSIPATEH.
