Struck by falling object

Employee was retrieving pry bar from storage area on the ground, when personnel above him lost control of new return roller they were changing out. The return roller fell, striking the employee on the ground. While the employee was wearing proper safety equipment at the time, the impact of the roller resulted in his death.

Employee States he is not sure how it happened, but felt discomfort in right eye Friday night, checked his eye, but could see anything, still felt discomfort in eye Saturday night, went to hospital and doctor discovered small piece of metal in eye, removed it, and employee is back to work at full duty

Employee was checking tire of welding trailer so it could be unhooked from vehicle. Left ""pinkie"" finger became pinched between tire of trailer & wheel chock, resulting in injury to pinkie finger, laceration to pinkie finger.

EMPLOYEE WAS CUTTING RUBBER CONVEYOR BELT MATERIAL WHEN UTILITY KNIFE EMPLOYEE WAS USING CAME FREE FROM MATERIAL HE WAS CUTTING, RESULTING IN A LACERATION OF HIS RIGHT KNEE.

Employee was operating crusher plant facilities, when he noticed that there appeared to be some spillage from one of the conveyor belts carrying material from crusher to stockpile, upon inspection saw that rubber skirting was out of place, Employee tried to push on rubber skirting with metal prybar, resulting in prybar getting caught between the conveyor and roller, kick back

CARRYING A PIECE OF STEEL ON FLAT GROUND STUMBLED & TWISTED RIGHT ANKLE CAUSING HAIR LINE FRACTURE
