Employee was placing screen decks (~40 Pounds) in storage rack when he felt a slight pain in lumbar region.

Employee alleges that he strained his lower back while pushing a toolbox around on the back of his Chevy 3500 flat bed mechanics truck.

Employee was carrying a bag of anfo on the stripped area, when he twisted and sprained his left ankle.

Employee was walking and supervisor drove next to him in pickup. The mirror hit employee causing in turn for the employee to hit the bed of the truck.

Stepped off ladder and twisted ankle.

While hammering off bearing, steel sliver punctured hand & lodged in his left hand, index finger joint.

EE was walking across an elevated steel mesh walkway to a control house, when he slipped and fell on the walkway. It was snowing and sleeting at the time. The mid-rail and hand-rail prevented the EE from falling to the ground.

Fixing a V-16. Pushing a pipe wrench to dislodge a belt, Belt loosened and moved, placing his hand between a piece of metal and the wrench handle.

While using pipe wrench to change v-belt on screen, wrench slipped, catching hand between wrench and structural iron.

Putting v-belts on v-16 pulleys and rolled fingers into pulley and v-belts and cut and smashed fingers.

EE HAD A WRENCH ON A BOLT, TIGHTENING BOLT, AS HE RAISED THE WRENCH, MUSCLE WAS PULLED IN HIS BACK.

PUTTING GRATES IN HAMMERMILL AND SMASHED RIGHT HAND ON GRATE.

UNLOADING WATER PUMP OUT OF TRUCK INTO BUILDING.

GRINDING PAOINT OFF OF IRON AND IN TIME, THE OIL BASED PAINT WORKED ITS WAY AROUND HIS FACE SHIELD AND GOT IN HIS EYES. WE WERE UNAWARE OF THIS REPORTABLE INCIDENT, UNTIL WE RECEIVED AN EMERGE NCY ROOM BILL ON MAY 15, 2000. UPON INVESTIGATION, THE EE HAD TOLD HIS SUPERVISOR IT WAS ALLERGIES, NOT RELATED TO HIS JOB.
