Injured employee and two other workers were replacing wear iron in Canica Impact Crusher when the hydraulically raised cover which was supplemented by support from a hydraulic jack which failed and knocked out the safety bar causing the lid to drop approximately 9" and rotate pinning injured employee's thigh against hand railing.

On 6/27/14 the EE indicated that the injury occurred on 6/20/2014. He explained that they had been lying on their backs tightening an adjustment nut for the belt with a wrench. It was an awkward position as they had to pull on the wrench at the same time to get the nit to tighten properly. He felt a little pain in his groin but he said it did not hurt that bad.

EE was lifting a screen which weighed approx. 50-60 lbs. up into the back of a pickup truck. As he was lifting the screen he felt his elbow tear. After about three days he noticed a significant amount of bruising in and around his right elbow. EE went to see his doctor Monday March 28th, and they set up an MRI and a follow up appt. for Friday April 1.

Employee reported some pain & discomfort in his left abdomen. We discussed it & we decided to rest his side after a weekend & see if the pain went away. The pain did subside. Then a couple of weeks later, he said the pain was back. He went to see the Dr. The Dr. put him on restricted.

Employee lifted a bucket of dirt to carry to his drill. When he turned he felt pain in his stomach muscles. He reported to me that he had a surgery to fix a hernia on 2-9-07 so this was the first time I verified that injury required surgery.

HE STOOD UP AND HIT HIS HEAD ON A CONVEYOR THAT WAS REMOVED OUT OF THE PLANT TO BE REPAIRED. THE IMPACT TO HIS SCALP CAUSED A CUT THAT REQUIRED THREE STITCHES.

EE WAS MOVING AIR TRACK DRILL AND STEPPEDN ON A ROCK WHICH CAUSED HIS KNEE TO TWIST. IT APPEARS HE TORE SOME CARTILEGE LOOSE IN HIS KNEE.
