While working on right drill pot, employee had hand laying on bolter sissor arm when right hand bolter man was told to move head causing employee's finger to be pinched between the bolter arms and bolter frame.
Employee was cutting in right hand x-cut when he for some reason advanced inby permanent roof support and was struck with a piece of falling roof.
While hanging belt chairs using a 6' step ladder ee fell from about 3 steps up ladder when ladder started to turn over from being set on loose material.
Pulling miner cable over out of roadway.
Lifting flight bar out of belt transfer, felt pain in abdomen/left side.
Backed miner up to clean up cut. Rock fell from between coal rib and roof bolts striking top of head. Knocked to ground - had four knots on head, knot & bruise on left shoulder. (Rock was 3' x 3' x 6"" in thickness.)
Lifting bucket of concrete mix overhead, felt pain in left arm and shoulder.
Laying interlocking block for seals & working on knees. Spilled concrete mix that got in employee's clothing and caused chemical burning on knees and shins.

Man slipped & fell striking hand on rock. Dislocated middle finger and small laceration.
According to foreman's report, injured stopped conveyer belt, when wooden crib block and rock came off belt, striking right knee.
Injured was pulling 3"" waterline when he slipped, causing waterline to hit him in the mouth, resulting in listed injuries.
AT BIRCH 2A PORTAL. EE WAS PULLING CABLE OFF OF REEL OF SHUTTLE CAR TO MAKE REPAIRS WHEN HE FELT PAIN IN HIS LOWER BACK AND LEGS. EE BEGAN MISSING WORK 5/6/05 AND OFFICE WAS NOTIFIED 5/16/05.
THE EMPLOYEE TWISTED HIS LEG GOING THROUGH THE MAIN DOOR & HE EXPERIENCED PAIN IN HIS GROIN. NOTE: INJURY REPORT WAS RECEIVED AT THE OFFICE ON 6/2/05.
Injured was removing a roof jack when a half header (wooden) fell from top of jack, approx. 8-10'high, striking him on the rt. cheek & causing a laceration. The laceration required 6 stitches. Jacks were going to be put into shuttle car to be moved to the next entry.
Injured was stepping off belt tailpiece when foot slipped off of rounded/wet 12""x12"" header causing injured to fall and hit back on corner of 12""x12"" header. Please note that this accident was not reported until May 2, 2005 because injured did not cease work until 5-2-05, due to injury worsening.

INJURED WAS WALKING ALONG BELT, WHEN A DRAW ROCK (3 1/2 FEET DIAMETER & 4"" THICK) FELL STRIKING HIM ON THE SHOULDER ON LEFT SIDE.
Injured was building a permanent stopping when a piece of draw rock fell from mine roof striking injured on right shoulder. Rock was 2' in diameter, and 2"" thick.
Injured was tramming scoop down an approximate 7% grade in roadway, and claims to have lost control of scoop and ran scoop bucket into rib, causing an abrupt stop, which in turn caused alleged injuries.
60 feet right of spad 663 #5 entry, draw rock fell between rib and bolts; rock was approximately 1 foot in diameter, and 3/4 inch thick; this rock struck injured in back.
Injured was hooking up chain behind ram on coal hauler when he stepped in hole, twisting knee and ankle, and also received abrasion to front of lower leg (shin).
WHILE SHOVELING MATERIAL & THROWING IT ON STACKER BELT TAILPIECE (CLEAN UP), EE CLAIMS TO HAVE EXPERIENCED SHARP PAIN IN LOWER BACK AREA.

LOADING SUPPLIES (RESIN) ON ROOFBOLTER, TWISTED AND PULLED MUSCLE IN LOWER STOMACH.
INJURED WAS TAKING UP SLACK IN BELT BY USING A GRIP/HAND HOIST WHEN ANOTHER EE ATTEMPTED TO HELP BY ""BUMPING"" BELT FORWARD WITH DRIVE MOTOR WHICH CAUSED INJURED'S HAND TO SLIP OFF HANDLE ALLOW ING IT TO STRIKE LEFT UPPER LEG. REPORT LOST DUE TO HOLIDAYS - OFFICE WAS CLOSED.
STEPPING OFF OF ROOF BOLTER MACHINE AND TWISTED RIGHT ANKLE #2 ENTRY.
LIFTING SHOVEL FULL OF MATERIAL, HE TURNED TO THROW IT INTO SCOOP BUCKET AND TWISTED LOWER BACK. (WAS NOT REPORTED WITHIN 10 DAYS BECAUSE PAPERWORK WAS LOST IN OFFICE MOVE).
MINER BACKED INTO FREEDOM CAR AND PINNED HIM BETWEEN CAR AND RIB. INJURED RT FOREARM.
UNINTENTIONAL ROOF FALL MEASURING 70'LONG, 20'WIDE, 4' TO 8'HIGH. OCCURRED IN NO.4 ENTRY 3 BREAKS OUTBY SPAD NO.322. NO INJURIES REPORTED. THIS ACCIDENT WAS CALLED INTO OAK HILL OFFICE BY MR. DALE LUSK.
