Employee was closing security gate and felt something give in his shoulder. He did not start to miss work until 9-11-2012.

Employee was removing a steering jack from a rock truck. As he was knocking the pin out, using a sledge hammer, his gloved right hand struck a protuding bolt resulting in a laceration that required stitches.
Employee was checking antifreeze on endloader when he lost his footing and fell onto catwalk spraining his left wrist on hand rail. A brace was applied.
Employee was stepping down from the steps of a 992G end loader, when he felt a pain that shot through his right leg. Diagnosis information regarding accident was not received until 1-26-2011, resulting in a torn meniscus.

EE fell down an embankment at the mine. He was taken to the hospital and no diagnosis was given. He was referred to a specialist for further testing. This report was delayed awaiting the diagnosis which was received on 11-15-10. He was told to remain off work until 12-01-10.

EE was climbing up the ladder at the back of Hydroseeder. The bottom ladder section swung up under the bumper causing EE to lose his footing and grip on the hand rail and fall landing on his back. He continued working until 11/23/2009.

While being loaded from a shovel a large rock rolled off the shovel and jolted the truck, causing ee to hit his knee on the retarder lever. Employee did not start missing work until 07/28/2008
A board holding the tarp on place fell when a gush of wind blew the tarp off the machine. The board hit him on the head.
EE was operating a loaded coal truck and was pulling out of pit #11, and because of the rain the roadway was muddy, and he spun out. A loader tried to push him but also spun out. The loader backed off and hit the truck several times while trying to get the truck up the hill. EE starting to have pain in his neck afterwards.

Pulling add-car of HWM from hole #604. EE pulled pin from #5 car, car came up off pad approx 4 1/2', then came down. EE did not release pin. Pin fell down causing two fingers to get under the lip on pin, causing amputation of left ring finer in front of 1st joint and mashing left middle finger.

EE assisting removal of rocks from HWM. He stepped back from miner, turned, and stepped off of walkway falling 65'. Taken to ER for treatment; injury consist of fracture left orbit socket, frontal sinus, & left forearm.
