IE WAS PUSHING COAL IN THE CLEAN COAL STOCKPILE AREA ABOVE NO.5 FEEDER. HIS DOZER SLID INTO A VOID THAT HAD FORMED IN COAL STOCKPILE. HIS DOZER WAS COVERED WITH COAL AND COAL ENTERED THE CAB VIA RIGHT SIDE GLASS.

The employee was hammering rock which was removed prior to it entering the vessel chain. A pressure switch (landmine) moved prior to this operation, weighing less than 10 pounds was being put back in place when employee stated employee felt back "pop".

EE was getting out of the excavator when he slipped and hit his left knee-also reported back pain. EE was using 3-point contact. EE did not seek medical attention until Friday 7.25.2014-RTW Monday 7.28.2014

Employee slipped and fell while traveling down a slope beside the truck dump hopper; twisted his right knee. He apparently has returned to his home in Oregon. We have not heard from him and we have no way to get in contact with him. Terminated as of this date (March 28, 2005).
