The employee was hammering rock which was removed prior to it entering the vessel chain. A pressure switch (landmine) moved prior to this operation, weighing less than 10 pounds was being put back in place when employee stated employee felt back "pop".

EE was getting out of the excavator when he slipped and hit his left knee-also reported back pain. EE was using 3-point contact. EE did not seek medical attention until Friday 7.25.2014-RTW Monday 7.28.2014
EE was feeding raw coal hoppers to plant with 988F end loader. EE stated that the brakes locked up throwing him forward into windshield.

EEG stated that appx. two weeks ago on 6.18.2013 he was climbing off the loader and stepped on uneven ground and twisted his left knee. NOTE: Reported incident after chosen for random alcohol/drug screening on 6.26.13. EEG has worked regular duties until 6.26.13--started missing work 6.26.2013

EE was driving 740 CAT Truck hit hole in road-seat bottomed out-EE felt pain in neck and back.
EE was doing preshift on impoundment when he slipped, his knee gave out. EE reported having pain in his knee on this day. EE has had knee surgery. EE missed NO work until he had surgery on 8.29.12
EE was pushing 12" pipe away from vessel with two other EE's- the left thumb was caught between pipe and I Beam-smashing the Left Thumb

Employee slipped and fell while traveling down a slope beside the truck dump hopper; twisted his right knee. He apparently has returned to his home in Oregon. We have not heard from him and we have no way to get in contact with him. Terminated as of this date (March 28, 2005).
