While installing a roof bolt, the roof bolt fell out of the top and struck employee in right thumb. Injury was not the result of a training or compliance issue. The incident was reported to the employer the next day.

EE was duck walking (walking in squatting position) in 43" coal, employee had a pain in employee's left side abdomen.

An unplanned roof fall occurred in the 2nd North West Mains, Spad # 815. The fall was appx. 40' Long 4' Thick. The fall is dangered off at all approaches.

EE hurt lower back moving a scoop tire.

An unplanned roof fall occurred in the 4th Panel of The North west Mains at spad 1350--The fall was 16' X 18' --4 1/2 ' Thick. The has been dangered off at all approaches.

Occupational Pneumoconiosis-Board findings-AWARD ORDER received on 4.18.2014 from AIG.EE has not missed work due to this illness. He is off work due to having knee surgery.

EE was splashed on the face with hot oil while moving it out the way.

EE WAS STEPPING OFF TAILPIECE AFTER GETTING ROCK UNSTOPPED AT THE HEAD AND TWISTED HIS LEFT KNEE. EE DIDN'T SEEK MEDICAL ATTENTION/OR MISS WORK UNTIL 2.7.2014.

EE fell to ground bruising the left elbow while working underground-finished shift.

EE was tramming 4-wheeler to section, hit a bump and hit head on mine roof.

While hanging shuttle car cable draw rock fell from between bolts and hit EE on left leg and foot. Rock size 40" wide 10' long and 6" thick.

EE was setting breaker up when it blew up causing burns to right side face, ear and Right hand.

EE was pulling slack from shuttle car cable to re-hang when he was shocked. PPE: EE was wearing rubber gloves.

EE was changing bits when draw rock fell out and struck him on head and shoulders.

Employee was putting up center lines crawling, he kneeled on left knee/twisted causing pain

EE was kneeling down on knees to check fire extinguishers-when getting up he felt a sharp pain in left knee.

EE was bolting top when a corner rib draw rock fell hurting his back. EE finished his shift bolting top--He went to see a Dr. on 6.21.13 to be checked-contusion on back. The EE has RTW FULL DUTIES.

EE was working on the bolter when the chain ratchet slipped dropping the scissors (arm Pot) on the left index finger causing a laceration that required sutures. EE missed one day to follow up with the doctor and has RTW modified duty until the sutures are out and laceration healed.

EE was lifting/moving a shield from the Miner then complained of his left shoulder hurting.

EE was getting back into the scoop when he put pressure on his knee it began to hurt.

EE was hanging cable in the #4 Entry-cable was pulled down, knocking him to the ground.

EE was loading structure in scoop bucket when he said he hurt his back.

EE raised a lid cover on the miner--it slipped and mashed the right thumb

EE was hauling coal to feeder with shuttle car, he hit his forehead on the fly pad, knocking his head back and cutting the back of head.

Mine Foreman was backing up his 3 wheeler ride when the miner cable caught the back of his head bent into chest causing neck and left shoulder pain. NOTE: EE did not miss work or seek medical attention until 7.23.12

EE was wearing protective knee pads that began rubbing blister on the right knee causing inflammation and redness. EE worked 3 hours on the 20th, could not finish shift, he is going to seek medical attention at Logan Regional.

This Incident happened after work in the parking area. Employee was showing a hand gun to co-worker, as he was putting his hand gun back in the car the gun accidentally discharged wounding himself in the left hand. He was treated at Williamson Memorial Hospital. The WV State Police investigated the incident and filed a report.

EE was standing behind manbus when the operator began to back up, hitting the EE left Foot.

EE was moving tail roller in place on the overland tail piece, smashed his left index finger with slate bar.

EE was driving the 2 man carrier when he lost grip of the steering wheel and it hit his right hand. EE finished shift doing regular duties.

EE was drilling outside hole and a piece of rock 6 ft long and 8 inches thick fell and hit him on the head, neck and left shoulder. Note: EE RTW on Friday and worked his regular duties no complaints, did not inform supervisor that he went to hospital. Began missing work Monday 1.30.12

EE was tramming miner across section, caught roofbolt with miner breaking bolt off. Bolt caught EE's left hand.

EE was loading glue onto roof bolter and twisted to load glue and hurt back.

Employee was loading cribs onto ride and twisted lower back-causing pain.

Employee was attempting to move a pump out of the roof bolter, while lifting he had a pain in his groin area. He finished his shift. He not lost any time due to his strain.

Employee was attempting to move belt, he was pulling using channel locks when he slipped off and fell back. He had pain in his lower right back.

Employee was installing roofbolt, the bolt hit a crack in the top and bent over and hit him in the right ear.

Employee was bringing the scoop outside, he raised his head and hit the top.

Employee was attempting to move a 20 lb charger for the mantrip. He had a sharp pain in the middle area of his back.

Employee was reaching across the mantrip-4 wheeler to get some tools, he said he must have hit a lever, the ride began to tram and rolled him against the top causing him shoulder pain.

Employee was bolting top-he was drilling and draw rock fell hitting him in the head, neck and back.

Employee was moving oxygen and acetylene tanks out of the roadway-felt a Pain in the bottom left side of stomach.

Employee was leaning forward checking for hydraulic leaks on the bolter. He was watching the hoses while moving the levers, the pot went up and caught his right arm causing a fracture.

Crew was making a power move, employee was pulling cables with a 4-wheeler when a piece of draw rock approx. 16 inches wide, 42 inches long and 2 inches thick fell out between bolts hitting the employee in the back of the neck, the coal seam in this mine is appx. 36 inches.

Employee was getting out of scoop and twisted his knee.

Employee was attempting to load belt rollers onto the ride to take underground; when he cut the bands on the pallet of the rollers they rolled off the pallet and hit his left foot and ankle.

Employee was backing up the ride, roofed out..hit his head on the mine roof.

Tramming bolter to another place, front end went in hole causing the back end to go into the top. Employee's neck and shoulder hit the roof top causing shoulder and neck pain.

EE was helping take shuttle car to the section. He was turning a ride around to get out of the way of the shuttle car. When turning the ride, his head jammed into the mine roof. EE stopped what he was doing and stated his neck was hurting.

Subject was taking a tire in for the mantrip, raised head up to catch the tire that was falling and caught his head in the roof.

A ROOF FALL MEASURING 80' L BY 18' BY 19' W BY 5' H WAS FOUND. IT WAS ON THE 002 SECTION IN #3 ENTRY NEAR SPAD #472.

EMPLOYEE WAS RATCHETING BELT TOGETHER AND PULLEDSOMETHING IN HIS BACK.

EMPLOYEE'S RIGHT FOOT WAS INJURED AFTER THE SCOOP THAT HE WAS OPERATING DRIFTED AND RAN OVER IT.HE HAD EXITED THE SCOOP TO HANG A LINE CURTAIN.

EMPLOYEE WAS FLIPPED BY A SHUTTLE CAR CABLE AS IT WAS TENSIONED.

EE WAS LOADING SUPPLIES FROM SCOOP BUCKET ONTO ROOF BOLTER. HE PLACED HIS HAND ON A SHARP METALSTRAP, LACERATING HIS PALM.

A ROOF FALL OCCURRED IN THE LAST OPEN BREAK FROMENTRY #3 TO #4. THE FALL MEASURED 80' LONG BY 19' WIDE BY 9-10' THICK.

LINE SPLITTER BURNT BLADE SWITCH.

EE WAS RIDING ON THE MANTRIP. THE OPERATOR BACKED UNDER A CABLE THAT WAS HUNG OVERHEAD. THE CABLE CAUGHT THE BOX CONTAINING SCSRS AND PUSHED IT OVER STRIKING EMPLOYEE.

EMPLOYEE WAS PREPARING TO SERVICE THE ROOF BOLTER, WHILE HE WAS STANDING BEHIND THE MACHINE, THE OPERATOR INADVERTANTLY AVCTIVATED THE CABLE REEL AND THE CABLE STRUCK EMPLOYEE IN THE ABDOMEN AND CHEST, KNOCKING HIM OVER.

EE WAS PULING ON A RIPPER MOTOR WHILE IN A BENT OVER POSITION.

EE PULLED THE LEVER TO LOWER THE CANOPY ON THE ROOF BOLTER. EE CLAIM THAT THE LEVER STUCK. THE CANCEL CAME .

UNINTENDIONAL ROOF FALL 001 1ST NORTHWEST MAINS (ABANDON WORKS) (#7 ENTRY) 80' ABOVE SPAD #125-17'TO 18' WIDE 160' LONG TOR 10' TO 20' HIGH.

EE WAS HANGING HI-LINE. WHILE TRYING TO STRAIGHTEN A HANGER WITH A PAIR OF CHANNEL-LOCKS, HE INJURED HIS MIDDLE FINGER ON HIS LEFT HAND. HE RECEIVED A PRESCRIPTION FOR THIS INJURY.

EE TWISTED TO PICK UP A BOLT AND HURT HIS BACK.

#7 ENTRY 1ST NORTHWEST MAINS. A FLAW IN TOP. FALL WAS ABOUT 160' LONG AND 7 TO 8 FT HIGH AND 18' TO 20' WIDE. STARTED AT BREAK SPAD #170 TO 30FT INBY SAPD #172.

EMPLOYEE STATES THAT HE WAS ATTEMPTING TO GET A ROLL OF FLYPOD MATERIAL FROM UNDER THE SUPPLY TRAILER, WHEN HE STRAINED HIS BACK.

EE WAS OPERATING SHUTTLE CAR WHEN HE STRUCK HIS HEAD ON A HEADER BOARD.

EE WAS OVER SCOOP BUCKET PUTTING A CABLE ACROSS IT WHEN OPERATOR HIT LEVER BY MISTAKE RAISING THE BUCKET AND PUSHING EE INTO ROOF

EE WAS HELPING MINER OPERATOR CHANGE BELTS ON MINER. EE WAS ON HIS KNEES EE MOVED & TWISTED HIS LEFT KNEE

EE WAS OPERATING SHUTTLE CAR WHEN HE DROVE THROUGH A HOLE. THE IMPACT OF STRIKING THE HOLE JARRED THE EE INJURING HIS LOWER BACK.

EMPLOYEE WAS WALKING INTO INTERSECTION AND WALKED INTO A BENT STRAP, CUTTING HIS RIGHT EAR.
