EE was wearing work gloves and dropped the fuel nozzle which landed on right 5th digit causing a laceration and fractured 5th digit. EE is being sent to hand specialist for medical treatment.

Employee, roof bolter, exited equipment and was struck by a rock off the bolted back. He received three stitches and bruises. Fifteen minute was notified and investigated by MSHA.

Equipment number MQ-0667 developed a hydraulic leak. Upon opening a spiral boom cover, hydraulic oil injected into the employee's left fifth digit.

Employee was changing a broken bucket cylinder. He was sprayed with hydraulic fluid on his body. While finishing the process of the job, hydraulic fluid dripped into his eyes. He was wearing proper personal protective equipment while the work was being done.

EE was hanging conveyor belt for an air wall using an air impact and noticed an irritation in his eye. EE was wearing all appropriate PPE including safety glasses.

Mulzer Crushed Stone Inc · Inhalation of radiations, caustics, toxic and noxious substances

EE was operating a CAT 325 Excavator with scaling tool when machine suffered a mechanical issue that resulted in exhaust/smoke. The operator was choked by the smoke causing coughing and gagging. He was sent to medical professional for evaluation which resulted in a prescription of Ibuprofen of 600mg thus making it recordable. Employee missed no lost days or restrictions.

Employee got out of Cat 325 scaler, after just scaling the back and a piece of limestone approximately 16 - 20 #s fell from the back and struck him on the shoulder. He stated that he was in serious pain, and was sent to the hospital ER for evaluation and treatment. Diagnosed with a contusion, and has already returned to work full duty. 1 Restricted day.

In the early am hours on 12-8-08 a roof fall occurred at the intersection of 9e + 9s below the anchorage point. In the early am hours on 12-9-089 more of the same area fell above the anhcorage point. The area has been blocked off and its abandoned. Reported to other ee on 12/10/08.
