HE WAS STANDING ON THE CONVEYOR COVER AT THE LOWER END OF #2 SCREEN. THE SCREEN SLID OUT TOO FAST AND HE TRIED TO CATCH THE SCREEN. HE LOST HIS BALANCE AND FELL 4 1/2 FT. TO THE CONCRETE BASE . OLD CONVEYOR CONTRIBUTED TO CRAMPED WORKING SPACE.

PERSON HIT ON THE END OF A STEEL LOADER PIN WITH A HAMMER. THE PERSON DID NOT HIT IT SQUARELY AND HE CHIPPED A PIECE OF STEEL OFF OF THE PIN. THE STEEL SHIP STRUCK HIM IN THE CHEEK OF THE F ACE CAUSING A CUT.
