Employee alleges employee was walking by the shop trailer and employee slipped on a belt that was covered by some snow causing employee to fall to the ground cracking 2 ribs.
Employee alleges he was getting down from the top of the screen and another employee was welding in his way and he hit his right elbow on the cab of the chute. Late reporting because we just received Dr. notes on 11/05/13 stating procedure was attempted but it didn't work. Employee didn't notify us of this.
Three employees had hooked up a battery charger to the batteries of a 988B endloader. Two employees were on the opposite side of the loader when they heard a noise. They went to the other side and found EE laying on the ground. They did not see what happened. EE doesn't remember anything about the incident or what he was doing.
