PICK UP WAS LOADED WITH FORKS FOR FORKLIFT. THEY EXTENDED BEYOND THE TRUCK BODY AND EE WALKED INTO THEM, CUTTING HIS FOREHEAD.
HE WAS REPLACING CUTTING EDGE ON 980G. WHILE LOOSENING BOLTS, ONE TORE INTO HIS GLOVE AND LEFT PLAM. THE LACERATION REQUIRED 9 STITCHES.
HE WAS USING A PRY BAR TO RELEASE A JAM. THE PRY BAR WAS CAUGHT AND SNAPPED BACK CATCHING HIS THUMB. HE HAD 7 STITCHES AND WILL PROBABLY LOOSE THE THUMB NAIL.

I. NAME STATED HE WAS BENDING OVER SHOVELING TO REMOVE ROCKS FROM A TUNNEL HE THEN EXPERIENCED SHARP PAIN IN HIS LOWER BACK. HE FINISHED THAT SHIFT AND WORKED THE NEXT DAY THEN HE HAS BEEN OUT OF WORK SINCE 7-1-88,
EMPLOYEE WAS GE7TING READY TO SWING THE SHOVEL ON THE BACKHOE WHEN THE BACKHOE TIPPED OVER A 3-4 FOOT EMBANKMENT ON ITS SIDE. DUE TO FROST CONDITIONS THE EMPLOYEE DID NOT NOTICE HOW CLOSE HE W AS TO THE EMBAN5MENT.
