Employee was trying to loosen a hydraulic hose on the boom of excavator when wrench slipped off smashing finger on boom causing a laceration (8) stitches and broken finger.

This was a fire contained to a wheel on tractor trailer coal truck. While setting in line to dump EE's load of coal it was noticed that the drivers off-side rear wheel area was on fire. It was fought with extinguishers and the local fire department was called to completely extinguish the flames.

Employee was loading a telehandler on a roll back when it slipped off jolting the employee in the cab. This resulted in small fracture to EE's upper shoulder.

Employee was in the process of securing a barge to be loaded, using a chain and clevis. The wind shifted and the barge moved up-river. EE had EE's hand on the chain when this occurred. The accident resulted in an amputation of EE's left index finger (amputated at local hospital).

Employee was cleaning the tipple when the metal detector shut the belt off. EE turned to go to the belt when EE placed EE's foot in some mud and EE twisted EE's knee. The area EE stepped in the mud was a small incline. EE had been scheduled for an MRI and management received notification on 2/2/2016 that EE will require surgery.

Employee was operating a loader when suspected dirt particles blew out of the heating vent and into her eye. She continued to work and completed her shift. Due to continued discomfort, she stopped by the ER and was diagnosed with a Corneal Foreign Body and prescribed antibiotic drops. Employee returned to work the following day.

Employee was removing lodged lump of coal with slate bar. In an attempt to free the material, the slate bar & lump fell out of the bin area striking employee's right leg ultimately causing a scrape on the leg. Employee refused medical attention initially, but went to family doctor on 5/26/09 because of redness. Doctor put employee off work for rest of week for infection.

Injured employee was in the process of dumping a load of coal off of his tractor/trailer when the trailer turned over, which in turn turned the tractor over. Injured employee's neck was lacerated during turn over. What caused the laceration was undetermined.

Employee picked up wash down hose to clean coal out of crusher. He was using his right hand to twist the nozzle to the on position. When the sudden burst of water pressure came on, the employee did not have a secure hold on the nozzle and the hose. This resulted in the employee's wrist being put in an awkward position, therefore, spraining his right wrist.

Employee tripped over a 3" toe plate while walking backwards giving hand signals to a crane operator. Employee fell on the existing catwalk bruising his knee, ribs, and shoulder. He was OK until a few days ago, decided to go back to the doctor on 9/15. Doctor put him on modified duty, but no modified duty is available, therefore it's a lost time.

Employee came into foreman's office and reported that his back was hurting and that he was going to seek medical attention. He informed his foreman that he had injured his back while pushing up loads of coal in the yard.

EE CLOSED A VALVE ON HYDRAULIC SYSTEM, CAUSING A BUILD UP OF PRESSURE TO A POINT ABOVE HYD HOSE PRESSURE LIMIT. THE HOSE BURST, HIGH PRESSURE OIL STRIKES ARM AND BODY CAUSING BRUISING AND ARM LACERATION.
