AN UNINTENTIONAL ROOF FALL OF A SLIP IN THE NO 1 ENTRY INBY CROSSCUT NEAR SPAD 180 MEASURED 34' L X 15' W X 5 1/2 H FELL ON THE ROOF BOLTING MACHINE AT 10:40 AM. THE AREA WAS BOLTED WITH 4' RE SIN BOLTS.

ON MAY 30, 2003, EE HAD EXITED THE TUNNEL WHERE THE BELT GOES UNDER THE ROAD. HE HAD HIS LUNCH BUCKET AND CLIP BOARD IN HIS LEFT HAND. AS HE STARTED DOWN THE STEPS HE SLIPPED. USING HIS RIGHT HADN, HE GRABBED THE HANDRAIL TO CATCH HIMSELF AND INJURED HIS RIGHT SHOULDER.
