TWO UNINTENTIONAL ROOF FALLS WERE DISCOVERED ON 5-9-03 THE FIRST FALL WAS LOCATED IN ENTRY NO. 6 NEAR SPAD NO 1405. THE FALL MEASURED 4 1/2' IN HEIGHT BY 20' IN LENGTH BY 20' IN WIDTH. THE SEC OND WAS IN ENTRY NO 7 NEAR SPAD NO 1429. IT WAS 4 1/2' H BY 20' W BY 20' LENGTH. BOTH FALLS HAPPENED APPROX 3:00 PM.

EE WAS LIFTING CABLE TO TOP AND FELT A STRAIN IN LOWER GROIN AREA. HE DID NOT MISS WORK OR SEEK MEDICAL ATTENTION UNTIL 3-23-03, IN WHICH I WAS NOTIFIED OF THIS ACCIDENT. ON 3-27 HE SOUGHT MED ICAL ATTENTION AND WAS GIVEN REST WITH NO LIFTING OR BENDING AND REFERED TO HIS FAMILY DOCTOR.
EE WAS RIDINBG MANTRIP OVER SIDE HE PLACED HIS FOOT ON SIDE POST OF CONPY WITH THE SIDE OF HIS FOOT OVER SIDE THE CONFINERS OF THE CAR. THE SIDE OF HIS FOOT CAME IN CONTACT WITH THE RIB FORCIN G HIS FOOT SIDEWAYS.

THE GLUT WAS PLACED ACROSS THE OPENING. WHEN THE GLUT WAS STRUCK BY THE AXE A PIECE OF THE GLUT FLEW UP & STRUCK EE'S FOREHEAD.

EE & CO-WORKER WERE CARRYING 48"TOP ROLLERS & PLACING THEM ON BELT RAILS. THE CO-WORKER WAS PUSHING THE TOP ROLLER ACROSS THE BELT RAIL AS EE WAS STEPPING OVER. EE STATES HE TWISTED HIS LEFT K NEE WHEN STEPPING OVER THE BELT. NOTE: EE BEGAN MISSING WORK FRIDAY AUGUST 30, 2002.

USING PICK HAMMER TO KNOCK OFF SLAG AND LOOSE METAL. WHEN THE METAL WAS STRUCK WITH A PICK HAMMER, A FRAGMENT OF THE HEAD OF THE HAMMER SHIPPED OFF AND PUNCTURED HIS LEFT FOREARM. FRAGMENT WAS EMBEDDED IN ARM. TWO STITCHES.

HE WAS INSTALLING STRUCTURE IN A BELT LINE. WHEN HE TWISTED AROUND TO PICK-UP ANOTHER PIECE OF STRUCTURE HE SPRAINED HIS KNEE. HE HAS BEEN OFF WORK SINCE THEN.
EE WAS HELPING MAKE A MOWER MOVE. HE WAS PULLING A S-CAR CABLE UP IN ORDER TO MOVE THE POWER CENTER. HE FELT PAIN IN HIS LOWER BACK. THE CREW TOOK A LUNCH BREAK. AFTER WHICH HE COULDN'T WORK S O HE WAS TRANSPORTED TO THE HOSPITAL AND HE HAS MISSED WORK SINCE.
EE WAS ARRANGING SUPPLIES ON TOP OF THE ROOF BOLTING MACHINE. HE WAS REACHING ACROSS THE BOLTER AND LIFTING A BUNDLE OF PLATES WHEN HE FELT PAIN IN HIS BACK.

EMPLOYEE WAS PLASTERING A STOPPING FROM THE #2 TO #3 ENTRY. A SHUTTLE CAR RAMMED INTO THE CROSSCUT AND STRUCK THE WHEELBARROW AT ROBERT'S RIGHTSIDE, PUSHING IT AGAINST ROBERT'S RIGHT LEG AND TWISTING HIS KNEE AS HE MOVED.

THE STRUCTURE SLIPPED AND FELL ON HIS BACK A ND SHOULDER. HE FINISHED THE SHIFT AND WENT HOME. AT ABOUT 0100 HE WENT TO THE ER. XRAY SHOW NO BROKEN BONE, BRUISING.
