TWO UNINTENTIONAL ROOF FALLS WERE DISCOVERED ON 5-9-03 THE FIRST FALL WAS LOCATED IN ENTRY NO. 6 NEAR SPAD NO 1405. THE FALL MEASURED 4 1/2' IN HEIGHT BY 20' IN LENGTH BY 20' IN WIDTH. THE SEC OND WAS IN ENTRY NO 7 NEAR SPAD NO 1429. IT WAS 4 1/2' H BY 20' W BY 20' LENGTH. BOTH FALLS HAPPENED APPROX 3:00 PM.

EE WAS LIFTING CABLE TO TOP AND FELT A STRAIN IN LOWER GROIN AREA. HE DID NOT MISS WORK OR SEEK MEDICAL ATTENTION UNTIL 3-23-03, IN WHICH I WAS NOTIFIED OF THIS ACCIDENT. ON 3-27 HE SOUGHT MED ICAL ATTENTION AND WAS GIVEN REST WITH NO LIFTING OR BENDING AND REFERED TO HIS FAMILY DOCTOR.

THE GLUT WAS PLACED ACROSS THE OPENING. WHEN THE GLUT WAS STRUCK BY THE AXE A PIECE OF THE GLUT FLEW UP & STRUCK EE'S FOREHEAD.

THE STRUCTURE SLIPPED AND FELL ON HIS BACK A ND SHOULDER. HE FINISHED THE SHIFT AND WENT HOME. AT ABOUT 0100 HE WENT TO THE ER. XRAY SHOW NO BROKEN BONE, BRUISING.
