CLIMBING UP INTO THE 988 MACHINE AND SLIPPED ON THE RUNG AND SLID BACK DOWN STRIKING HIS CHEST AND RIGHT KNEE.

EMPLOYEE WAS SPLITTING A PIECE OF GRANITE WHEN A PIECE OPENED AND DROPPED ON HIS RIGHT FOOT PINNING IT UNDER THE STONE AND FRACTURING HIS BIG TOE. EMPLOYEE DID NOT HAVE ON HIS SAFETY BOOTS.
