While holding box-end wrench to help loosen bolt and nut, the impact gun being used by coworker on the bolt spun the wrench out of hand of the victim striking EE's middle finger.

Employee's foot slipped off of ladder, in grabbing to stabilize self, EE cut EE's middle finger and palm on the metal guard. EE is at work, did not miss any days but is on restricted duty.

Employee was using channel locks to attempt to pull a bad lace from the conveyor. As the lace broke free, the momentum caused the employee to fall forward and employee struck mouth on the conveyor frame damaging six teeth. Dental work required to repair the teeth.

Claiborne hauling Co. truck driver, operating truck # C-212, came onto site at 10:30 am needing to wash bed out. Preceded to wash station entered into an adjacent area and proceeded to raise bed into high voltage line.
