SLIPPED ON ICE COVERED STEPS, CAUSING TWISTING OF LOWER LUMBAR AND DISLOCATION OF LITTLE FINGER ON LEFT HAND. STEPS HAD BEEN CLEARED AND TREATEDTWICE PRIOR TO EMPLOYEE FALLING. FREEZING RAIN MOST OF THE PREVIOUS NIGHT AND DAY OF ACCIDENT,
ONE SECTION OF FLOOR GRATING HAD BEEN REMOVED TO CLEAR MAGNETITE AUGER AND CHECK VALVE ON MAGNETITE SEPARATOR. EE WENT TO GET TOOL AND STEPPED IN AREA WHERE FLOOR GRATING HAD BEEN REMOVED. FEL L APPROX. 2 FT HITTING KNEE ON IN PLACE GRATING AND STRADDLED PIPE EE WAS INSIDE OF ROPED OFF AREA.
