VICTI WAS STANDING AT REAR OF ROOF BOLTER, EMPLOYEES HAD PARKED MAC-8 APPROX 30' AWAY AT TOP OF SLOPE. APPARENTLY PARK BRAKE HANDLE SPRUNG UP TO THE OFF POSITION AND MAC-8 ROLLED DOWN SLOPE AND STRUCK VICTIM, PINNING HIM AGAINST ROOF BOLTER.

WHILE SHOVELING ON #2 BELT, FOUR BREAKS INBY #2 HEAD DRIVE THE SHOVEL TWISTED BETWEEN THE ROOK &THE BELT WHILE HE WAS EMPTING THE SHOVEL. THIS CAUSED THE SHOVEL TO KICK BACK POSSIBLY INJURING HIS RIGHT ELBOW & TWISTING HIS WRIST AREA & CAUSING POSSIBLE STRAIN TO HIS BACK.
