MINER WAS MINING CROSSCUT LEFT OFF #2 ENTRY (100' INBY SPAD #356) WHEN IT CUT INTO WATERHOLE USED FOR STORING WATER FOR MAIN PRESSURE PUMP. MINE MAP WAS OFF AT LEAST 1 BREAK (60'-100'). MINER AND SHUTTLE CAR WERE COVERED WITH WATER. NO MEN WERE INJURED.
