Laceration to thumb requiring stiches. During LOTOTO, employee stepped into help with project installing v-belts on CV7 conveyor. EE pulled the belt, and it got caught on EE's thumb.

EE was operating a 988K PL0105 wheel loader, digging in the muck shot and sending loads to Primary Crusher. As EE was digging in the muck pile, EE hit a tight spot which caused the loader to jar/jolt, and the EE strained EE's lower back.
EE was installing a right-side, A motor V-Belt guard on a HP400 Crusher. EE hung a 44" X 37", 20lb piece of guard onto the fastener bolts. As EE was lining up the bottom right-side fastener, the guard came unhooked from top left fastener. This caused the guard to swing down and pinch EE's left hand between the guard and angle support, bruising EE's left hand.
Miner was shoveling stone and sand off floor in tunnel and putting it on conveyor belt. After shoveling, EE said had pain in the back of the right shoulder.
EE was washing screen tower walkways with a 1-1/2" rubber water hose. EE finished washing off plant and disconnected hose from water line 1-1/2" camlock couplers. When removing hose fitting from pipe water sprayed out hitting them in the left eye. EE washed the eye but still felt discomfort, was seen by a panel physician and has an abrasion on the cornea and received prescription.

EE was walking down a slight incline along the side of a roadway, EE slipped on loose stones and fell forwards onto EE's knees as EE fell, EE caught self with EE's right arm spraining EE's right shoulder and causing a contusion on both knees

Employee was operating a Volvo A35D Articulated truck when the truck slid in a tire rut. The operator had right hand on the wheel and left hand on the grab rail. The sudden movement jerked employee's body to the right causing pain in left shoulder.

WHILE CLEANING OUT PRIMARY CRUSHER, EE HAND SLIPPED OFF ROCK CAUSING HIM TO FALL ON ROCK AND FRACTURE HIS RIBS.
EMPLOYEE WAS CLEANING BIG ROCKS FROM THE CATWALK AFTER A SPILLAGE. A ROCK SLIPPED OUT OF HIS HAND AND LANDED ON HIS FOOT BEHIND HIS SAFETY SHOES.
