AS HE DID THIS HE PULLED A MUSCLE IN HIS BACK. THE WEATHER WAS VERY COLD AND MAY HAVE CONTRIBUED TO HIS INJURY. EE WAS OFF 3 DAYS.

EE REFUSED TO SEE A DR BUT FINALLY CONVINCED HIM TO GO. EE WAS TAKEN TO A LOCAL DR AND HAD STITCHES PUT IN. THE DR RELEASE EE BACK TO WORK.

CONDITIONS WERE DAMP AND FINER MATERAIL WAS BEGINNING TO COLLECT ON THE SIDE BELT CHUTE OF THE SCREENING PLANT. EE WAS USING 6' WOODEN HANDLED SCRAPER TO CLEAR THE MATERIAL BUILD UP. THE SCRAP ER BECAME WEDGED. EE PUT PRESSURE ON HANDLE AND IT BROKE. THE HANDLE HIT HIM IN THE FACE CAUSING A LACERATION TO HIS CHIN. EE RECEIVED SUTURES. AND RELEASED.

EE WAS LOADING TOVEX INTO THE 1987 1/2 TON PICKUP. AS EE WAS GETTING OUT OF THE BACK OF THE PICKUP HIS RIGHT FOOT SLIPPED CAUSING A FALL. EE BRUISED THE INSIDE OF HIS LEFT LEG. EE WENT TO THE DR AND THE DR FOUND A BLOOD CLOT FORMING ON THE INSIDE OF HIS LEFT LEG JUST BELOW THE KNEE. THE CONDITIONS WERE WET & MUDDY. THE EMPLOYEE HAS JUST PUT ON COMPANY WIDE SAFETY PROGRAM ON THE HAZ
EE WAS HELPING WITH A WATERPUMP INSTALLATION. EVERYTHING WAS COMPLETED AND EE WAS STARTING TO LOWER THE SUCTION HOSE, AS HE LOWERED THE HOSE THE WINCH HANDLE SLIPPED OUT OF HIS HAND THUS LETTI NG THE WEIGHT OF THE HOSE SPIN THE HANDLE. INSTEAD OF LETTING THE HOSE FALL EE TRIED TO CATCH THE HANDLE RESULTING IN A FRACTURED FINGER AND CUT ACROSS THE LEFT HAND. CONDITIONS WERE WET AND

INJURED PERSON WAS SERVICING LOADER. WHILE ATTEMPTING TO ADD HYDRAULIC OIL TO RESERVOIR ON LOADER TRANFER HOSE CAME OUT OF OPENING STRIKING HIM ON KNEE.
THE INJURED PERSON AND 2 OTHER MEN WERE ATTEMPTING TO MOUNT A BOOM ON AN ENDLOADER BUCKET. WHILE TRYING TO ALIGN THE PIN HOLES, THE BOOM SLIPPED OFF THE BUCKET STRIKING THE INJURED PERSON IN T HE LEFT LEG.
WEATHER WAS CLO6DY AND MISTING HE WAS SEVERICING IMPACT BREAKER WAS WALKING UP CONVOYER BELT AND SLIPPED STRICKING HIS LEFT ELBOW ON PEICE OF METAL COVER. NO LOSS TIME ON 4-17-86 HIS LEFT ELB OW BECAME SORE 5ND INFLAMED AND WENT TO DOCTOR.
