Before they knew it the left steer tire was already over the left berm embankment as they were exiting the pit just before the scale. Resulting in the truck and pup to roll onto its left side.

Loader operator returned to his loader after lunch and turned on the air conditioner. When the air conditioner fan turned on, it blew out some dust and small debris into his face. Employee flushed out his eye and returned to work. The next day his eye was still irritated so he went to the doctor and was treated for a scratched cornea and prescribed antibiotic eyedrops.

Bin truck driver twisted left knee at the end of his shift. Saw Dr. on 5-20-10 and was released back to duty driving truck. 5-27-10 follow up with Dr., MRI referral and restricted duty thru 6-1-10. 6-1-10 follow up with Dr., referral to orthopedic surgeon and relased back to duty driving truck. Will update 7000-1 following appointment with orthopedic surgeion on 6-21-10.

12-6-07 employee slipped on ice straining his left knee. Employee did not seek medical treatment. 12-11-07 employee reported that knee was not improving, supervisor took him to local clinic. Employee was examined, diagnosed with knee strain, released same day, told he could continue to operate water truck, but should undergo 4-8 sessions of physical therapy.

Employee stated that his back was sore. He does not know why. Employee visited a doctor on 12/16/2005. Doctor suggested that short term physical therapy may help.
Control Room Operator strained lower back while lifting a jackhammer. Operator did not notify this office of the injury until October 3, 2005. Employee went to his doctor. Employee was released back to work the same day with lifting restrictions and referred to PT. PT says that restrictions will not affect the employee's ability to perform his normal routine duties.
