THE HEATER LOCATED IN THE CONTROL CAB MALFUNCTIONED CAUSING A FIRE. THE OPERATOR ATTEMPTED TO SHUT OFF THE HEATER AND FAILED. THE CONTROL CAB AND ITS CONTENTS BURNED IN THE FIRE.

HE REACHED UP TO GRAB ONTO THE RUNG OF THGE LADDER TO START CLIMBING UP TWO OR THREE STEPS TO GET THE HYDRAULIC MOTOR THAT OPERATES THE LEGS OF THE CRUSHER.AS HE REACHED UP AND BEGAN TO PULL H IMSELF ONTO THE LADDER,HE FELT A SHARP PAIN BETWEEN HIS ELBOW AND HIS RIGHT WRIST.
