Omega Highwall Mining, LLC · Caught in, under or between a moving and a stationary object

Employee was removing a pin from the right angle of the Highwall Miner with a pry bar. The pry bar kicked back from the tension and pinning EE's left hand on the tail chain and striking EE on the head.

Employee was using a slate bar to remove pins from the right angle conveyor as the pin was dislodged, the shifting weight of the conveyor caused the slate bar to pivot upwards, causing the slate bar to strike him on the right cheek.
