Shelly Materials Inc · Over-exertion in lifting objects

Miner injured back lifting screen 7/9/13. Said he was fine didn't need medical treatment. Pain got worse so 7/22 took him for MRI. Results came 8/5 showing new damage to back. Put on light duty.

WORKER WAS REMOVING SCREEN BOLTS FROM SHAKER WITH A AIR WRENCH WHEN A BOLT THAT WAS WORE DOWN REAL SHARP SPUN ACROSS HIS FINGER AND CUT IT OPEN.HAD TO HAVE FINGER STITCHED UP, 4 STITCHES.

EE WAS CLEANING OUT CHUTE AT FEED HOPPER ON #3 DRY PLANT AND FELL 5'ONTO THE GROUND & LANDED ON A LARGE ROCK ON HIS LEFT SIDE. HE WORKED THE REST OF THE WEEK AND WAS O-K, SATURDAY HE WAS TAKEN TO THE HOSPITAL AND THEY FOUND HE HAD BROKEN HIS SPLEEN.

EE AND ANOTHER EE WERE TRYING TO CHANGE THE TRIPCABLE. INJ EE WAS ATTEMPTING TO PUT THE CABLE BACK INTO THE CROWS FOOT WHEN IT SLIPPED OUT OF HIS HAND AND HIT HIM IN THE FOREHEAD. EE HAD TO HAVE 9 STITCHES DO TO HIS LACERATIONS.
