THE WAND OF THE SPRAYER HUNG UP ON A HYDR AULIC HOSE. THE POWER WASHER WENT THROUGH HIS BOOT. THE HEATED WATER ENTERED HIS BOOT AND BURNED THE BOTTOM OF HIS FOOT GIVING HIM 2ND DEGREE BURNS.

EE HAD FINISHED FUELING VEHICLE AND WAS ATTEMPTING TO ENTER OPERATOR'S COMPARTMENT. HE PUT HIS FOOT ON THE TOP STEP OF SCRAPER WHEN HIS SLIPPED HE FELL BACKWARDS TO THE GROUND. EE STSATED THA T HIS BOOTS WERE MUDDY AND HIS GLOVES WERE WET. EE SUSTAINED A FRACTURED WRIST.
A DRILL AT STAMPER QUARRY CAUGHT ON FIE. EFFORTS BY EE TO EXTINGUISH THE FIRE WERE UNSUCCESSFUL. THE FIRE DEPT WERE CALLED AND PUT OUT THE FIRE. THE CAUSE OF THE FIRE ARE UNKNOWN AT THIS TIME.
THE EMPLOYEE WAS REMOVING COLD WEATHER CLOTHING ON THE WALKWAY OF THE DRESSLER HAUL-PAK HE IS ASSIGNED. THE FRONT END LOADER OPERATOR SHOOK THE VEHICLE WHEN HE BEGAN TO LOAD CAUSING HIM [INJUR ED] TO FALL APPROXIMATELY 6 FEET.
EE WAS TRAINING TO DRILL WITH OTHER HE NOTICED A CRACK ON THE COUPLER. HE THEN SHUT DOWN TO INFORM HIM HE GEGAN TO REPAIR THE DRILL AS EE WATCHED. EE REMOVED HIS SAFETY GLASSES TO WIPE THE SWE AT FROM HIS FOREHEAD HE STARTED TO BACK UP THE DRILL STEEL CAUSING A ROCK TO BECOME AIRBORNE. AND STRIKING EE IN THE RIGHT EYE.
LOADER WAS BEIN1 SERVICED AND THE OPERATOR SLIPPED AND FELL FROM TIRE. THE RESULT WAS A SMALL HAIR LINE FRACTURE OF THE LOWER LUMBAR AREA TO ONE VERTABRA.
