Opened engine compartment on loader and did not latch the cover. Cover slipped out of the miner's hand and hit him across the nose and knocked him to the ground.

EMPLOYEE WAS USING THE FIRE HOSE FROM THE WATER TRUCK, TO WASH OUT THE MUD FROM THE TRANSMISSION BELLY PANS OF TWO HAUL TRUCKS. THE NOZZLE END WAS HELD IN PLACE BY A HOSE CLAMP. THE HOSE BECAM E SEPARATED FROM THE NOZZLE. THE HOSE SLIPPED THROUGH HIS HAND AND THE HOSE CLAMP CUT HIS THUMB.

EE WAS CLIMBING INTO THE CAB OF THE EXCAVATOR. THE DOOR WAS LATCHED IN THE OPEN POSITION. EE UNLATCHED THE DOOR TO CLOSE IT BEHIND HIM. AS HE WAS CLOSING THE DOOR THE WIND CAUGHT THE DOOR & IT CLOSED ON HIS FINGER IN THE DOOR JAM.

EE WAS POWERWASHING MUD AND GREASE OFF OF THE TRANSMISSION THROUGH THE INSPECTION PLATE ON THE CATWALK OF LOADER. HIS FOOT WAS IN THE INSPECTION HOLE. THE WAND OF THE SPRAYER HUNG UP ON A HYDR AULIC HOSE. THE POWER WASHER WENT THROUGH HIS BOOT. THE HEATED WATER ENTERED HIS BOOT AND BURNED THE BOTTOM OF HIS FOOT GIVING HIM 2ND DEGREE BURNS.

THE EMPLOYEE WAS REMOVING COLD WEATHER CLOTHING ON THE WALKWAY OF THE DRESSLER HAUL-PAK HE IS ASSIGNED. THE FRONT END LOADER OPERATOR SHOOK THE VEHICLE WHEN HE BEGAN TO LOAD CAUSING HIM [INJUR ED] TO FALL APPROXIMATELY 6 FEET.

EE REMOVED HIS SAFETY GLASSES TO WIPE THE SWE AT FROM HIS FOREHEAD HE STARTED TO BACK UP THE DRILL STEEL CAUSING A ROCK TO BECOME AIRBORNE. AND STRIKING EE IN THE RIGHT EYE.
