INJURED ENTERED THE BIN AFTER BEING WARNED 30 MINUTES PRIOR NOT TO WORK IN THE AREA.APPARENTLY HE BECAME ENTRAPPED BETWEEN THE CONVEYOR AND THE BIN DISCHARGE GATE AND SUFFOCATED.

THERE WAS A FAULT IN THE SECONDARY SYSTEM. THE GROUND WIRE WAS BURNED IN TWO AND THE CONVEYOR FRAME WAS ENERGIZED. AN EMPLOYEE WAS ELECTROCUTAL WHEN HE TOUCHED THE FRAME. GROUND WAS WET AROUND THE AREA.
