HE HAD JUST LOADED UP ON THE CUT AND WAS COMING DOWN THE RAMP TO UN-LOAD.THE OPERATORS WITH SCRAPERS HAD BEEN INSTRUCTED TO LOWER THE SCRAPER PANS WHEN GOING DOWN GRADE.HE DROPPED IT TOO MUCH AND STOPPED THE TRACTOR,HE THEN RAISED THE PAN BUT DID NOT LOWER IT AGAIN.THE WEIGHT OF THE LOAD IN THE SCRAPER CAUSED THE TRACTOR TO JACK-KNIFE AND TO UPSET OFF THE EDGE OF THE RAMP.HE WAS TR

EE WAS OPERATING A CAT AND SCRAPER AT DAGGETT QUARRY. AFTER EE WAS LOADED, HE BEGAN TO HEAD DOWN ON THE FILL TO UNLOAD. THE CAT TRACTOR DROVE UP OVER A LARGE CHUNK OF FROZEN GROUND. EE HAD HIS SEAT BELT IN PLACE. SUDDENLY HE WAS THROWN FORWARD, CAUSING HIS BACK TO POP. THE SAFETY DIRECTOR OBSERVED THE OPERATIONS AND FOUND THE FILL TO BE SMOOTH AND IN ORDER.
