Cutting little finger on left hand. Shutting cab door on drill.

Stepped out of cab to get off of machine that EE was measuring. Turned to position self to step down backwards & grab grab-iron. This is the motion that tweaked EE's back. EE was in the parking lot not in the mine itself. EE has prior back issues from past injuries.

Employee was grabbing a spade from the dozer operator and didn't keep mind on task. The spade stuck his lower lip and had to have 2 stitches. Returned to work with no lost time.

AT APPROX 5 AM ON SEPT 3, 02 THE EXPLOSIVES STORAGE MAGAZINE EXPLODED. I FIRST HAD KNOWLEDGE OF THE EXPLOSION AT 6:00 AM AND CALLED MSHA AT APPROX 6:20 AM CST. MAGAZINE WAS LOCATED IN UNION AT THE THAYER QUARRY.

EE WAS FUELING DOZER AND WHILE ROLLING HOSE BACK ON TO THE FUEL TRUCK REEL THE CELENAID ON THE SWITCH STREET AND THE HOSE CONTINUED TO ROLL CATCHING THE EE LEFT RING FINGER AND BREAKING THE ON E AT THE TIP.
EE WAS WORKING UNDER LOADOUT FEED BIN #220. TIGHTENING A BOLT. WHILE COMING OUT FROM UNDER EQUIPMENT HE STOOD UP TOO SOON & HIT HIS BACK JUST BELOW THE NECK BETWEEN SHOULDER BLADES ON CROSS MEMBER

THE CONDITIONS AT ATLANTIC QUARRY WERE WET AND MUDDY. EMP WAS HOOKING CHAIN TO BACKHOE BUCKET. MUD WAS EVERYWHERE INCLUDING ON BOOTS. FEET CAME IN CONTACT WITH WET STEEL OF LOADER BUCKET. THIS CAUSED A SLIP AND EMP LOST BALANCE. SLIP CAUSEDEMP TO PULL MUSCLES IN RIB AREA. EMP WAS OFF WORK 1 DAY.

EE WAS HELPING CHANGE A CABLE ON A CAT DOZER. AS HE WAS PULLING OLD CABLE OUT, HE CLAIMED HIS ABHOMINAL MUSCLES WERE SORE. EE WAS BOWLING IN A LEAGUE WHEN HE WAS OFF. I AM CONCERNED IF HE WAS ABLE TO COME BACK TO WORK SOONER.
