DRY GRAVEL FLOOR ING NATURAL AND CLEAR CONDITONS PERSON BENT OVER TO LIFT GARAGE DOOR. GARAGE DOOR OPENED ONE FOOT AND STUCK. ADDED EFFORT TO PUSH DOOR PAST STICKING POINT RESULTED IN LOWER BA CK PAIN.

THE OPERATOR OF TENSILE MACHINE TRIED TO TURN OFF EQUIPMENT. UPON REACHING FOR THE SWITCH,THE LEFT PINKY FINGER SLIPPED BETWEEN SAFETY GUARD FOR COOLING FAN. LOSS OF 1/8" OF FINGERTIP AND THE FINGERNAIL. INADEQUATE GUARDING AND OPERATOR INAATTENTIVENESS WERE THE PRIMARY CONDITIONS WHICH LED TO THE ACCIDENT.
