Employee was loading holes, he reached out to his right, to grab a 50# bag of ANFO and felt a sharp pain in his lower back. He strained his lower back.

INJURED WAS TAKING A PART LOSE WITH A CHISEL ANDIT SLIPPED BACK, STRIKING HIM NEAR THE RIGHT EYECUTTING THE SKIN. THE IMPACT CAUSED HIM TO SEE BRIGHT LIGHTS AND HAVE SOME BLURRED VISION.
