11 recordedEE stated he had been to #3 rock bin to inspect the chute of #4 refuse belt. Witness: EE was on the 1st or 2nd step of a small ladder w/backward fall protection when he "blacked out".*He slipped from ladder landing on feet & fell on his back*. He never lost consciousness. We are waiting for medical records to determine the cause.*On 12/18/07 he had surgery & he died on 12/19/07*
