EMPLOYEE WAS MUCKING OUT THE PILE WHEN HE NOTICED WHAT HE THOUGHT WAS A MISS-FIRE.HE THEN DUG HIS WAY TO THE SUSPECTED MISS-FIRE,WHICH PLACED HIM AND HIS MACHINE UNDER LOOSE MATERIAL.THE EMPLO YEE THEN EXITED HIS MACHINE TO RETRIEVE THE SUSPECTED MISS-FIRE.WHILE STANDING AT THE FACE,A LARGE ROCK SLIPPED OFF THE FACE,CRUSHING THE EMPLOYEE.

EMP TAKEN OFF LIFE JACKET ATTEMPTED TO SWIM AFTER BOAT WHICH HAD DRIFTED OFF FROM THE BANK. WITNESS ADVISED THAT HE STOPPED SWIMMING AND WENT UNDER THE WATER ONCE AND CAME BACK UP. EMP THEN W ENT UNDER THE WATER AGAIN AND DID NOT COME BACK UP
