THE VICTIM WAS OPERATING THE MATERIAL HAULER WITH ANOTHER EE, AS A PASSENGER. AS THEY WERE TRAVELING DOWN AT A 5-8% GRADE, THE OPERATOR APPARENTLY TOOK THE MACHINE OUT OF GEAR TO TRAVEL MORE Q UICKLY. HE HAD TROUBLE GETTING THE MACHINE BACK IN GEAR. PASSENGER JUMPED SAFELY. TEH TRIP PROCEEDED 300 FT DOWN THE ROAD. THE OPERATOR EITHER JUMPED OR WAS THROWN AND RUN OVER BY THE EIMCO.

HE WAS DROPPING AND GOT IN UNDER THE WHEELS. IT HAD BEEN RAINING AND WAS MUDDY AND SLICK.THE CORONER PRONOUNCED HIM DEAD AT THE SCENE.

HE TOOK A GUARD OFF NO. 2 BELT LINE TAKE-UP FOR WHAT REASON WE DO NOT KNOW, RESULTING IN HIM BEING CAUGHT BETWEEN TAKE-UP STATIONARY ROLLER AND BELT, RESULTING IN A FATAL ACCIDENT. HIS JOB WAS A BELTMAN.

VICTIM WAS ATTEMPTING TO NEGOTIATE A TURN ON THE SHUTTLE CAR, AND IN DOING SO, HE ATTEMPTED TO SEE BETTER BY REMOVING HIMSELF FROM UNDER THE CANOPY AND WAS TRAPPED AND CRUSHED BETWEEN THE RIB AND THE CANOPY.
