I NAME AND I N AME ROOF BOLTERS WERE GOING TO INSTALL A SPOT BOLT PRIOR TO PINNING THE CUT TWO SAFETY JACKS WERE SET I NAME WAS INSTALLING A THIRD JACK WHEN A PIECE OF DRAW ROCK 7X7X4 FELL ST STRIKING HIM IN THE HEAD AND SHOULDER RESULTING IN SERIOUS HEAD INJURIES
