EMPL HAD BEEN INSTALLING ROOF BOLTS IN #1 ENTRY. HE WAS DISCOVERED BY COWORKERS LYING ON BACK NEAR CONTROLS OF MACHINE & UNDER PERMANENTLY SUPPORTED ROOF. SECTION OF ROOF MEASURING 12'LONGX7.5 'WIDEX1" TO 12" THICK HAD FALLEN INBY PERMANENT ROOF SUPPORTS. THERE WAS NO ROCK ON VICTIM.
