Employee was digging out the sump pond near pump #1 with an excavator. While dumping the last pile, the rocks bounced off of each other and struck the front of the excavator, shattering the right side glass. This glass then struck the member on the hand and wrist causing a small (approximately 3") laceration to left wrist.

The member was sweeping stairs inside of the scale house when a truck arrived requiring attention. Member turned towards the console and missed the bottom step, losing balance, and landed on EE's hip and side.

Injured employee was assisting an additional employee lift a 4x8x1/8" steel plate weighing approximately 160 lbs when employee alleged experiencing pain in shoulder. Employee was transported to urgent care facility by Plant Superintendent for evaluation. Injured employee issued work restriction by urgent care to remain out of work until evaluated by orthopedic.

While using a utility razor knife to cut rubber skirting for conveyor belt, worker cut thumb when knife slipped. Worker was treated at a medical facility and received sutures.

WORKER CUT HIS FINGER WHEN THE RAZOR KNIFE HE WAS USING TO CUT A RUBBER HOSE SLIPPED. MEDICAL TREATMENT INCLUDED 5 SUTURES. WORKER RETURNED TO WORK THE NEXT DAY AT FULL DUTY.

Worker was air arcing the inner main frame liner out of the crusher (while wearing a welding mask) when an airborne debris particle flew into his eye. The particle became imbedded and had to be removed by a doctor. The doctor also gave him a prescription.

EMPLOYEE WAS LIFTING THE PLANT FEEDER MOTOR TO REMOVE A BELT WHEN THE HOOK ON THE HAND OPERATED LEVER HOIST BROKE. HE WAS STRUCK IN THE HEAD BY THE PORTION OF THE HOOK THAT WAS STILL ATTACHED TO THE HOIST CABLE. HE RECEIVED A LACERATION THAT REQUIRED 5 SUTURES.
EMPLOYEE WAS REMOVING A CRUSHER CONCAVE RING. THE SUPPORTING PLATE WAS DISLODGED AND STRUCK EMPLOYEE IN FOREHEAD. THE EMPLOYEE RECEIVED A LACERATION THAT REQUIRED SUTURES.
WHILE OPERATING A CRANE USING A DROP BALL TO BREAK LARGE STONES,EMPLOYEE WAS STRUCK ON LEFT KNEE BY A STONE FRAGMENT, HE FRECEIVED A LACERATON THAT REQUIRED SUTURES.
