On 7/27/2017, it was reported our company by Industrial Hearing Testing, a company engaged to evaluate hearing test results that, an employee had incurred a reportable hearing loss of which the employee was notified. (Employee was retested on 8/30/2017) Employee was placed on the MSHA Log, but this was overlooked and not reported at the time.

Worker was using a grinder to prepare metal to be welded when an airborne metal particle flew into his eye and became imbedded. Worker was wearing safety glasses. The particle had to be removed by a doctor.

While shoveling stone during installation of new pipe between 2 settling ponds, worker experienced low back pain. He was able to return to his regular job as a truck driver after one missed day of work.
