EE was reversing an L350H front end loader down the ramp that leads to the primary crusher. As the EE was reversing, EE cut the turn too close to the berm that lines both sides of the ramp. EE's right tires went on top of the berm, causing the FEL to tip over on its left side. EE sustained a dislocated left shoulder, and there was heavy damage to the left side of the FEL.

Employee was replacing a feeder plate. He had a 3/4 electric impact gun on a shelf above were he was working. When he reached over to put a crescent wrench on a bolt he caught the cord of the impact which pulled it off the shelf. It struck his left thumb.
Employee was operating a skidsteer loader when he hit a bump and the seat bottomed out. This employee did not seek medical treatment until August 31. His first lost time day was August 31, 2010.

Slipped while dismounting front-end loader causing a wrist sprain when he caught himself from falling. NOTE: This claim was originally contested. It was ruled a workplace accident by Ohio BWC on January 25, 2010.
On top of truck repairing drive for auger unit on truck. Employee was running drive to check alignment on sprocket and chain. The unit was stopped so he could tighten set screw on key way for sprocket. The control was bumped and auger started to run, catching his index finger on left hand.
