Miner was cutting a piece of belting off of a roll when the knife slipped and the Miner cut EE's left hand in two places. The laceration resulted in two sutures.
Employee was working on Canica crusher in C5, and was under the belt tunnel removing covers to get to the belts and sheave when a piece of metal got under EE's safety glasses and into EE's eye.

Miner was torching on old storage bin and heard a pop, fell to ground and injured right knee.
Employee burnt left forearm on take-up that was hot when trying to adjust for better position to cut bearing off PC3 tail pulley.
Employee had finished loading pellet truck and flipped switch off to stop flow of material and notify trucker. Employee turned to right when they fell injuring left knee and right wrist.
Employee was putting bucket pin on a front end loader when it fell out of the pin boss smashing their left pinky finger against the boom arm.

While cleaning material on secondary screen tower, a rock rolled down and struck employee's shin.
Employee was removing glass breaker from front end loader when it came loose and they accidentally struck the injured worker in the mouth, chipping two front teeth.
Employee was at the wash sand screw and went to step over a pipe but a block was there. EE stepped on the block and fell, catching self with left hand. EE did not report the incident to management until 3/8/24.

Lost footing and fell backwards striking a metal corner.
Employee reported ongoing tingling in left wrist along with soreness and weakness from repetitive motion over time. Diagnosis of carpal tunnel syndrome
Employee injured left index finger while attempting to bend a piece of metal with a chipping hammer. EE missed and struck the end of finger. This resulted in EE losing the fingernail.
Miner was in the pit and responded to loader operator reporting coolant issue. Miner went to put coolant in the loaders and when miner went to crack the cap the hot pressurized coolant sprayed the miner in the neck and upper torso.

Using come-along to hang pulley on conveyor, additional chain hanging inside come-along needed to be moved. When chain was moved it tightened come-along and caught middle finger between chains.
EE was using a long drift pin, to remove bolts that were stuck. As they hit the pin with a hammer, they instantly felt something and saw that thumb was bleeding. EE didn't believe they struck themselves and couldn't see anything in the thumb. EE believes a piece of metal from either the drift pin or the hammer entered left thumb at the top, just behind the knuckle.

Miner was doing preventative maintenance on an air separator. When EE took EE's last step up onto the platform EE felt a "pop" in EE's left knee.
Employee was mounting front-end loader and stepped over a mudpuddle to avoid getting feet wet. Strained lower back when employee pulled self onto ladder.

While cutting a piece of belting for a patch with a utility knife, the knife slipped resulting in a laceration to the right thigh. **It was found during internal audit that this report was not submitted within 10 working days of accident and is being submitted now on 1/28/2020.
Employee was cutting a piece of rubber belting with a utility knife when the knife slipped striking left forearm. Small laceration of left forearm. Wound required one suture to close making it reportable.
Pulling sticks from the trailer, right shoulder popped
While backing dust truck up ramp to unload, miner got too close to right side berm and truck rolled over onto driver's side.
Lifting and pushing screen head resulted in hernia in abdomen. Returned to work full duty after missing 1 day. Potential for surgery in the future.

Employee was closing the lid on a covered hopper when the hook slipped causing EE to pinch finger between the hook and handrail. Fracture/Laceration of right middle finger.

Smashed his fingers between the door and the lock while opening a rail car. A small laceration was closed with Steri-Strips and blood was drained from under the fingernail of his middle finger on his right hand.
Employee was operating a front end loader when he ran over a rock. When the loader bounced he hit his head on the roof. He received a laceration on the left side of his head which needed sutures.
Employee was unplugging the Canica Crusher when a rock fell from the top of the crusher and hit his knee. Was diagnosed with a sprain/strain of the knee.
Employee was walking when he turned his ankle on uneven ground.
Employee experienced gradual onset of carpal tunnel syndrome. This was diagnosed in April but not reported to the company until October 1. Medical confirmation was received Oct. 15. Company believed this was not reportable until first day of lost time.
Employee claimed that he developed pain in his shoulder and neck region over a period of time while performing housekeeping duties. NOTE: This claim was originally declined for untimely reporting and an unspecified mechanism of injury. Ohio BWC recently ruled that this injury was work related.

Employee was cleaning snow from between the railroad tracks using a skid-steer loader when his bucket caught a splice in the rail, causing the loader to stop suddenly, injuring his back. Employee returned to work on 01/09 and then went back off work on 01/21.

Employee was standing on a platform over the top of the rail line inspecting when a car that was too tall struck the platform. The platform was pushed over by the car. The employee suffered a contusion of the chest wall and a left knee sprain and abrasion.

The employee tripped over a frozen rut while walking from parking area to the garage.
The employee lifted a lid off the 20's bin and felt a pain in his back. Injury was a sprain/strain. This was not a reportable accident until January 21 when the employee missed work.

Employee was lifting lid off turbulator when his hand was pinched between the lid and surrounding framework. Result was a contusion of the right hand.
A trespasser came onto National Lime and Stone property and committed suicide by setting fire to his vehicle while inside it. This fact was confirmed by the Wyandot Cty. Sheriff and the Ohio Fire Marshals Office.
Employee was reporting for work. After checking with the train operators, he attempted to climb over two coupled train cars when he slipped and fell, injuring his left hip and elbow.

Carpal Tunnel Syndrome - Left Wrist This condition was reported to the employer on 1/20/05.
Pinched his arm between the chain and hook while lifting a conveyor take-up. This caused three small cuts requiring stitches to close.
While attempting to lift chute into place the chute flipped over, striking him on the shoulder. Emplyee received stitches in his upper left arm.
Employee was cleaning belt conveyor walkway when he felt pain in his right knee. MRI shows torn meniscus.
Shoulder started to hurt while working on screen.
Employee walked into a slide gate on a chute. His prescription eyeglasses struck the gate which caused a laceration on his eyebrow. Wound needed stitches to close.
In was discovered on this date that this employee has a possible work related hearing loss. This report was discovered during a self audit on 7/23/04.
